经胸超声心动图在复合型先天性心脏病封堵术中的应用

摘　　要：目的:探讨经胸超声心动图在复合型先天性心脏病封堵术中的应用价值。方法:应用经胸超声心动图对29例适合封堵术的复合型先天性心脏病患者行经导管封堵术。房间隔缺损合并肺动脉瓣狭窄、室间隔缺损、动脉导管未闭分别是10例、3例、5例,室间隔缺损合并动脉导管未闭1例。结果:29例患者成功进行了封堵,术中及术后均无并发症,封堵器固定良好,无移位,无残余分流,合并PS者,跨肺动脉瓣平均压差由术前(52.6±20.5)mm Hg(1 mm Hg=0.1333 kPa)下降到术后(14.3±9.5)mm Hg。结论:复合型先天性心脏病封堵虽较单纯型复杂,技术要求相对较高,超声心动图能对多种心血管结构改变进行诊断,从而指导临床医生对手术方式、预后等情况进行准确评估,可获良好的治疗效果。Objective To investigate the transthoracic ultrasound heartbeat graph in composite occlusion of congenital heart disease application.Methods Application of transthoracic ultrasound heartbeat graph on 29 cases for plugging operation of combined congenital heart disease patients undergoing transcatheter closure.Atrial septal defect associated with pulmonary valve stenosis,ventricular septal defect,patent ductus arteriosus were 10 cases,3 cases,5 cases,ventricular septal defect combined with patent ductus arteriosus in 1 cases.Results 29 patients underwent successful closure,intraoperative and postoperative complications were well fixed,occluder,no shifting,no residual shunt,combined with PS,across the pulmonary valve average pressure difference from preoperative（52.6＋20.5）mm Hg（1 mm Hg=0.1333 kPa） down to the postoperative（14.3＋9.5）mm Hg.Conclusion combined congenital heart disease Amplatzer although relatively simple complex,technical requirement is relatively high,ultrasound heartbeat graph can be a variety of cardiovascular changes in the structure of the diagnosis,so as to guide the clinician to the operation mode,and for accurate assessment of prognosis,can obtain good therapeutic effect.
