应用智能化用药安全警示互动系统干预肾内科药物不良事件效果评价

摘　　要：目的:评价临床药师应用智能化用药安全警示互动系统(简称互动系统)在预防药物不良事件(ADEs)等方面的干预效果。方法:以肾内科为研究科室,运行互动系统,收集ADEs,统计警示信息,采用配对方法研究ADEs对住院费用及时间的影响;采用前后对照方法研究干预前后ADEs发生率变化情况。结果:应用互动系统之后,警示信息数显著下降,药师与医师互动率达35.3%,医师采纳率达80%;比较干预前组(2008年8月～2009年7月)和干预后组(2010年12月～2011年9月),标准化率得可预防ADEs发生率从8.2%下降至0.8%,预计每1000人可减少74例可预防ADEs,从而减少共约1 057 016元费用和999天住院时间。结论:互动系统作为一种新的干预手段,不仅有利于临床用药安全,降低可预防ADEs,也有利于发挥与培养临床药师的职责。Objective：To evaluate the effect of ＂Intelligent Interactive Alerting System for Drug Use＂（referred to as IS） applied by pharmacists on reduction of adverse drug events（ADEs）.Methods：IS was deployed in the nephrology department,then ADEs and alerts were collected monthly,the excess length and extra cost attributable to ADEs by matched case-control study were recorded,and the changes of the incidence of ADEs with the self-control method were studied.Results： After the intervention of IS,the number of alerts significantly decreased with an interactive rate of 35.3%between physicians and clinical pharmacists and an acceptance rate of 80%in physicians.The incidence of preventable ADEs（pre-ADEs） of the test group（August 2008 to July 2009） decreased from 8.2%to 0.8%,which was much more effective than that of the controlled group（December 2010 to September 2011）.It was estimated according to the standardized,rate that about 74 cases per 1000 persons could be prevented with a total reduction of 1 057 016 yuan and 999 days.Conclusion：IS, as a new intervention could not only improve drug treatment and reduce the incidence of pre-ADEs,but also exert or cultivate clinical pharmacists＇ responsibilities.
