摘  要:目的探讨外周血淋巴细胞减少能否提示重型甲型H1N1流感。方法分析2009-2010年经病原学确诊的21例重型甲型H1N1流感患者血常规结果和其他特点。结果 21例重型患者中,除2例(9.5%)合并细菌感染者外周血白细胞升高外,其他19例(90.5%)外周血白细胞均正常或降低;17例(81.0%)外周血淋巴细胞比例<20%,且绝对计数均<0.8×109/L,最低值分别为1.2%和0.1×109/L。随着病情改善,除1例外,其余16例淋巴细胞计数在病程2~3周内恢复正常。结论外周血淋巴细胞比例和绝对计数下降可提示重型甲型H1N1流感。Objective To study the association between lymphopenia and severe pandemic H1N1 influenza A infection. Methods Routine blood tests and other clinical data were analyzed for 21 patients with severe pan- demic H1N1 influenza A infection that was confirmed by reverse transcription PCR during 2009 and 2010. Results All patients, except two complicated with bacterial infections, were detected with normal or reduced white blood counts. Seventeen (81.0%) patients had decreased lymphocyte percentages (〈20% ) and counts (d0.8)〈 109/L.) with the lowest at 1.2% (0.1)〈 109/L). After 2 to 3 weeks of treatment from the disease onset, both lymphocyte percentage and count returned to normal level or a level slightly higher than normal in 16 out of the 17 patients. Conclusions Lymphopenia might be an indicator of severe pandemic H1N1 influenza A infection.
