北京地区多民族高校社区2型糖尿病眼底病变的临床分析

摘　　要：目的:探讨高学历多民族社区2型糖尿病视网膜病变(DR)的相关影响因素。方法:对社区内管理的326例2型糖尿病患者(T2DM)进行眼底照相,结合患者的病程、血压、血糖、糖化血红蛋白、血脂等指标进行分析。结果:在326例T2DM患者中,大学及以上学历271例(83%),少数民族211例(65%),汉族115例(35%)。DR72例(22.08%),其中单纯性DR33例(45.83%),增殖性DR(PDR)39例(54.17%)。DR患者与非糖尿病视网膜病变患者(NDR)比较在病程、血压、餐后2小时血糖、糖化血红蛋白、甘油三酯、胆固醇、低密度脂蛋白等方面比较,差异有显著性(P<0.05);而在空腹血糖、高密度脂蛋白等方面比较,差异无显著性(P>0.05)。结论:2型糖尿病视网膜病变的发生与糖尿病患者的病程长、血压、血糖、血脂等指标控制不良相关,是今后糖尿病社区防治的重点。Objective：To analyses the related factor of the development of diabetic retinop- athy（DR） in type 2 diabetes mellitus. Methods：All 326 cases with type 2 diabetes mel- }itus in our community were evaluation with the method of the fundus photography and anal）sed retrospectively with the course of disease,blood pressure, level of glycosylat- ed haemoglobin （ HbA1c ）, fasting and post- prandial blood sugar,blood lipid and so on. Results：Among the 326 cases with the type 2 diabetes mellitus, patients with DR were found 72 cases（22. 08% ）. Including 72 ca- ses, 33 cases （45.83%） were the simple DR,and 39 cases（54.17% ） were the prolifbrative DR. Compared with the NDR pa- tients, the levels of blood pressure, glycosy- lated haemoglobin （ HbAl~. ）, postprandial blood sugar, Triglyceride （ TG ） , cholesterol （TC） , and low density lipoprotein - choles- terol （ LDL - C ） of the DR patients were no- table different （ P 〈 O. 05 ）. But there were no difference in the levels of high density lipoprotein - cholesterol （ HDI. - C ） and fasting blood glucose. Conclusion： DR mor- bidity is related to the long course of diabe- tes patients, abnormality levels of blood pressure, HbAlc,postprandial blood sugar, TG, TC and LDL - C. So we shoud pay more attention about prophylactico -therapeutic measures of diabetes in the Community.
