情绪障碍儿童的家庭动力学特征

摘　　要：目的探讨情绪障碍儿童的家庭动力学特征。方法采用方便取样方法，选取在南京医科大学附属脑科医院儿童心理卫生研究中心确诊的情绪障碍儿童74例，同时，以性别、年龄相匹配抽取南京市普通学校的正常儿童148例。采用系统家庭动力学自评问卷研究儿童的家庭动力学特征，用自编一般情况调查表调查儿童的性别、年龄、父母的婚姻状况、父母的职业、家庭经济水平、家庭类型等情况。结果病例组家庭动力学自评问卷的家庭气氛、系统逻辑2个维度评分[分别为（30．54±8．28）分，（17．14±4．26）分]高于对照组[分别为（23．45±7．40）分，（15．43±3．86）分]，差异有统计学意义（P〈0．01）。Logistic回归分析结果显示，高个性化（OR=0．903，95％CI：0．834-0．977）、高疾病观念（OR=0．853，95％CI：0．750-0．970）是儿童情绪障碍发病的保护性因素，而高家庭气氛（OR=1．167，95％CI：1．101。1．236）是儿童情绪障碍发病的危险因素。结论情绪障碍儿童的家庭动力学特征表现为“沉闷、敌对”的家庭气氛，“非此即彼”的家庭系统逻辑。家庭气氛、疾病观念、个性化程度与儿童情绪障碍发生相关。Objective To investigate the family dynamic characteristics of adolescents with emotional disorder. Methods 74 adolescents with emotional disorder from Child Mental Health Research Center of Nanjing Brain Hospital Affiliated to Nanjing Medical University were chosen and compared with 148 genderand age matched controls from schools in Nanjing. Self-rating scale of systemic family dynamics（SSFD） was used to assess the characteristics of family dynamics, and general demographic information inventory was conducted for all sub- jects. Results Adolescents with emotional disorder showed higher scores in family atmosphere and systemic logic than control group（ （30.54 ±8.28） vs （23.45±7.40） ,（17.14 ±4.26） vs （15.43 ±3.86） ,all P〈0. 01 ）. Logistic regression analysis showed that high individuation （ OR = 0. 903,95 % CI ：0,834 ± 0,977 ） and high disease thinking （ OR = 0. 853,95 % CI ：0.750 ± 0. 970）were protective factors for emotional disorder. High family atmosphere （ OR = 1. 167,95% CI ：1. 101 - 1. 236） was risk factor for emotional disorder. Conclusion Adolescents with emotional disorder demonstrate boring and hostile family atmosphere and ＂Either/or＂ family systemic logic on the family dynamic characteristics. Family atmosphere, disease thinking and individuation are associated with emotional disorder.
