摘 要:目的:探讨复灌Ⅰ号注射液对大鼠肢体缺血-再灌注损伤骨骼肌钙调蛋白表达的影响。方法:54只SD大鼠随机分为3大组,分别为对照组(control组)、缺血-再灌注组(IR组)和复灌Ⅰ号药物干预组(Ⅰ-IR组)。对照组所有大鼠不阻断血流;IR组和Ⅰ-IR组复制右后肢缺血-再灌注模型。对照组动物处死前不给药物,缺血-再灌注组于造成缺血前12h、造成缺血时和再灌注时于腹腔内各注射生理盐水1mL;复灌Ⅰ号药物干预组于造成缺血前12h、缺血时和再灌注时腹腔各注射复灌Ⅰ号注射液1mL。各组动物于再灌注0h、2h、4h、8h等时间点在右后肢相应区域切取约100mg腓肠肌,冰盐水快速洗去血液,滤纸吸干水份,剪碎制成组织匀浆,用于兰尼啶受体(ryanodine receptor,RyR1)和肌浆网钙泵(sarcoendo-plasmic reticulum Ca2+-ATPase,SERCA1)的RT-PCR检测。结果:⑴骨骼肌型RyR1mRNA的RT-PCR结果:Ⅰ-IR组在缺血2h时表达最强,但要比IR组为弱,2组比较差异有统计学意义(P<0.01),8h时表达都降低,虽然Ⅰ-IR组降幅要比IR组为小,但其差异有统计学意义(P<0.05)。⑵SERCA1mRNA的RT-PCR结果:复灌Ⅰ号注射液干预各组的表达也随时间延长而减弱,但下降幅度较IR组为小,与IR组相比,除2h时外,在0h(P<0.05)、4h(P<0.01)、8h(P<0.01)等时间点其差异均有统计学意义。结论:复灌Ⅰ号对缺血-再灌注损伤中骨骼肌的保护作用可能是通过双向调节RyR1以使早期减轻胞浆钙超载,后期保护细胞器的作用来实现的。这可能是其防治肢体缺血-再灌注损伤,减轻骨骼肌损伤的主要分子机制之一。Objective:To study the influence of Fuguan InjectionⅠ on calmodulin expression in skeletal muscle of ischemic-reperfusion injury rats.Methods: Fifty-four rats were divided into 3 groups at random: control group,ischemic-reperfusion group(IR group) and Fuguan injection Ⅰ intervention group(Ⅰ-IR group).The rats in control group did not undergo blockade of blood flow in posterior limbs.The ischemic reperfusion models of rats in rats' posterior limbs in IR group and Ⅰ-IR group were established by the method suggested by Zhang Lianyuan.No drug was given before sacrifice in control group.One ml normal sodium was intraperitoneally injected 12 hours before ischemia,at ischemia and at reperfusion in the IR group.One ml Fuguan InjectionⅠwas intraperitoneally injected at the same time points as in IR group.Zero h,2 h,4 h,and 8 h after reperfusion,100mg of gastrocnemius muscle was taken from the right posterior limb respectively in the three groups for RT-PCR test of RyR1 and SERCA1.Results: ⑴The skeletal muscle type RyR1 mRNA expression reached its highest level(lower than IR group) inⅠ-IR group 2 hours after ischemia,indicating significant difference between Ⅰ-IR group and IR group(P0.01).RyR1 mRNA expression decreased(lower than IR group) inⅠ-IR group 8 hours after ischemia,also showing significant difference between Ⅰ-IR group and IR group(P0.05).⑵Within 8 hours after reperfusion,SERCA1 mRNA expression was gradually down-regulated both in Ⅰ-IR group and IR group,with less decrease in Ⅰ-IR group.Except for the 2-h time point,there were significant differences between IR group and Ⅰ-IR group(P0.01).Conclusion:Fuguan Injection Ⅰcan protect skeletal muscle during ischemic reperfusion injury.The reasons for this may be through the two-way regulation of RyR1 mRNA expression to decrease calcium overload in the early time while to protect cell organ in the later time.And this may be the main molecular mechanism of decreasing the injury of the muscle in limb du
