摘  要:目的通过对老年男性慢性心力衰竭(CHF)患者血浆总睾酮、超氧化物歧化酶(SOD)、丙二醛水平及左心室质量指数(LVMI)、左心室舒张未内径(LVIDD)的观察,探讨男性CHF患者血浆睾酮水平与心脏重构及氧化应激的相互关系。方法选择男性CHF患者(CHF组)50例,另选年龄相匹配的男性患者(对照组)40例.检测血浆睾酮、丙二醛和SOD水平,应用超声心动图测量LVMI、LVIDD,进行对比和直线相关分析。结果与对照组比较,CHF组睾酮、SOD水平明显降低(P<0.01).丙二醛、LVIDD、LVMI明显增加(P<0.01);CHF组睾酮水平与LVMI、LVIDD、丙二醛呈负相关(r=-0.732、r=-0.629、r=-0.623,P<0.01);与SOD水平呈正相关(r=0.743,P<0.01);SOD水平与LVMI、LVIDD呈负相关(r=-0.621、r=-0.594,P<0.01)。结论男性CHF患者睾酮下降可致心室重构加剧及机体氧化应激水平提高。同时机体抗氧化物质的减少对心室重构有重要促进作用。Objective To study the correlation of plasma testosterone level with ventricular remodeling and oxidative stress by observing the plasma testosterone, superoxide dismutase(SOD) and malondialdehyde(MDA) levels,left ventrieular internal diastolic diameter(LVIDD) and left ventrieular mass index(LVMI) in aged male patients with chronic heart failure(CHF). Methods Fifty male CHF patients served as a CHF group and 40 age-matched male patients served as a control group. Their testosterone,MDA and SOD levels were measured and their LVMI and LVIDD were calculated on ultrasound cardiogram with their linear correlation analyzed. Results The tes- tosterone and SOD levels were significantly lower while the MDA level, LVMI and LVIDD were significantly higher in CHF group than in control group(P〈0.01). The testosterone level was negatively related with the MDA level, LVMI and LVIDD(r = - 0. 732, - 0. 629, and - 0. 623, P〈0.01) and positively related with the SOD level(r=0. 743,P〈0.01) while the SOD level was negatively related with the LVMI and LVIDD(r=-0. 621,-0. 594,P〈0.01) in CHF patients. Conclusion The decreased testosterone level in male CHF patients can promote the ventricular remodeling and increase the oxidative stress by decreasing the antioxidant level.
