老年女性冠心病合并慢性肾脏病3期患者的临床特点分析

摘　　要：目的分析老年女性冠心病合并慢性肾脏病(CKD)3期患者的临床特点。方法将111例老年冠心痛患者按性别及CKD诊断标准分为女性冠心病+CKD 3期组(A组)32例、女性单纯冠心病组(B组)49例,男性冠心病+CKD3期组(C组)30例,对患者临床情况、生化指标、心脏结构功能、动态血压进行对比分析。结果与B组比较,A组患者白蛋白明显降低(P<0.05),脉压虽有升高,但差异无统计学意义(P>0.05)。与C组比较,A组患者糖化血红蛋白、TC、TG、LDL-C明显升高(P<0.05,P<0.01),血红蛋白、左心室质量指数明显降低(P<0.05,P<0.01),LVEF虽有升高,但差异无统计学意义(P>0.05)。结论冠心病合并CKD3期老年患者中女性血脂、血糖水平高于男性,左心室质量指数低于男性,老年女性冠心病合并CKD 3期即可出现白蛋白下降及24 h脉压增加。Objective To analyze the clinical characteristics of aged female patients with coronary heart disease（CHD） accompanying stage 3 chronic kidney diseases（CKD3）. Methods One hundred and eleven aged patients with CHD were divided into female CHD＋CKD3 group（group A, n=32）,female CHD group（group B,n=49）,and male CHD＋CKD3 group（group C,n=30） according to their gender and CHD diagnostic criteria. Their general clinical condition, biochemical parameters, cardiac structure and function, and dynamic pulse pressure（PP） were analyzed. Results The albumin（ALB） level was significantly lower in group A than in group B（P〈0.05）. No significant difference was observed in PP although it tended to increase （P 〉 0.05）. The TC, TG, LDL-C and HbA1C levels were significantly higher while the ALB level and left ventricular mass index（LVMI） were significantly lower in group A than in group C（P〈0.05）. No significant difference was found in PP although it tended to increase（P〉0.05）. Conclusion The blood glucose and lipid levels are higher while the LVMI is lower in female CHD patients than in male CHD patients. The ALB level decreases while the 24 h PP increases in aged female CHD patients complicated with CKD3.
