家兔低血压状态下静脉注射多巴胺致TNF-a、IL-6的表达及局部病理改变的研究

摘  要:目的观察家兔在低血压状态下静脉注射多巴胺引起炎症反应的情况,为临床安全用药提供实验依据。方法将72只家兔随机分为A组:静脉滴注生理盐水;B组:静脉输入20mg多巴胺加100ml生理盐水;C组:低血压状态下静脉输入20mg多巴胺加100ml生理盐水。分别于用药后0.5h、1h、2h、4h采集血液标本测定血清炎性因子浓度,同时取长0.25cm、宽0.25cm的组织做病理学观察。结果三组TNF-a、IL-6在0.5h、1h时间点比较,差异无显著意义(P>0.05),2h、4h时间点比较,差异有极显著意义(P<0.001)。病理学改变三组比较总阳性率差异有极显著意义(P<0.001)。结论低血压状态下静脉注射多巴胺较正常血压下炎症反应出现时间早、程度重,提示临床护士应高度重视低血压状态下静脉注射多巴胺引起的组织损伤。Objective To observe the expression of inflammatory factors with intravenous dopamine for rabbits with hypotension state and provide experimental basis for safety clinical use of dopamine. Method 72 rabbits were divided randomly into group A (saline) ; group B (saline + dopamine) and group C (saline + dopamine in hypotension state). The inflammatory factors concentration were determined with collecting blood samples after 0.5 h, 1 h, 2 h, 4 h. Result The TNF-a, IL-6 of three groups have no difference in 0.5 h and 1 h (P〉0.05). On the contrary, it has more differences in 2 h and 4 h (P〈0. 001). There was no significant differences in 0.5 h-4 h in group A (P 〉0.05), but there was significant difference in 0.5 h-4 h in group B and C (P〈0. 001). The total positive rate of pathological changes in the three groups was significant is different (P〈0. 001). Conclusion Intravenous dopamine can result inflammatory reaction in rabbits in no leakage, and the inflammatory responds appears early and heavily in hypotension state. It is suggested that our clinical nurses should attach great importance to patients with tissue damage by intravenous dopamine during hypotension and adopt the measures of intervention synchronously.
