三七通络汤联合西药对变异型心绞痛患者动态心电图的影响

摘　　要：目的:探讨三七通络汤联合西药对变异型心绞痛患者动态心电图的影响,分析动态心电图在变异型心绞痛诊断和疗效判定中的价值。方法:70例符合纳入标准的变异型心绞痛患者,分别给予常规心电图和动态心电图检查并根据就诊先后顺序,用随机数字表法随机分为两组。对照组30例,采用消心痛片治疗;治疗组40例,在对照组基础上加用三七通络汤治疗。两组均14 d为1个疗程,2个疗程后统计疗效,分析动态心电图在变异型心绞痛诊断和疗效判定中的价值。结果:①动态心电图在S-T段抬高、T波高耸、室性早搏、U波倒置、房室传导阻滞及异常Q波的检出率等方面均明显高于常规心电图(P<0.05);②两组动态心电图疗效比较:治疗组有效率为97.50%,对照组有效率为83.33%,χ2=4.390,P<0.05,治疗组优于对照组;③两组症状疗效比较:治疗组有效率为92.50%,对照组有效率为73.33%,χ2=4.755,P<0.05,治疗组优于对照组。结论:动态心电图在变异型心绞痛诊断中阳性检出率明显高于常规心电图,其S-T段抬高的程度和阵次可作为变异型心绞痛疗效判定的主要参考条件。Objective：To explore the effects of integration of western medicine and Sanqi Tongluo Decoction on dynamic electrocardiogram of patients with variant angina pectoris,and to analyze the valuation of dynamic electrocardiogram on the diagnosis and curative effect determination of variant angina pectoris.Methods：70 cases of variant angina pectoris meeting the inclusion criteria were respectively given the examination of routine electrocardiogram and dynamic electrocardiogram,and then were divided into two groups by the method of random number table according to subsequence of treatment.The control group（30 cases） was treated by Isosorbide Dinitrate Tablets;the treatment group（40 cases） was additionally treated by Sanqi Tongluo Decoction based on the treatment given to the control group.Curative effects of both groups were analyzed after two courses of treatment with one course of 14 days and the valuation of dynamic electrocardiogram on the diagnosis and curative effect decision of variant angina pectoris was analyzed.Results：① Determination ratio of dynamic electrocardiogram was obviously higher than that of the routine electrocardiogram in the aspects of ascending of ST,towering of T wave,premature ventricular contractions,inversion of U wave,atrioventricular block and detection rate of abnormal of Q wave,P0.05;② Curative effect of dynamic electrocardiogram in the treatment group was 97.50% while that in the control group was 83.33%,χ2=4.390,P0.05.The treatment group was superior to the control group;③ Curative effect of symptom improvement in the treatment group was 92.50%,while that of the control group was 73.33%,χ2=4.755,P0.05.The treatment group was superior to the control group.Conclusion：The positive detective ratio of variant angina pectoris diagnosed by dynamic electrocardiogram is obviously higher than routine electrocardiogram,whose ascending degree and beats times of ST part can be considered as the main references for the determination of curative effect of variant angina pector
