IgA肾病抗核抗体阳性的临床意义

摘　　要：目的探讨抗核抗体(ANA)在IgA肾病中的阳性率及其对临床的意义。方法回顾性分析2007年1月-2009年12月间进行ANA检测的115例IgA肾病患者及59例非IgA肾病慢性肾炎患者的临床资料。统计分析ANA阳性与阴性的临床指标。结果 IgA肾病患者中有12例(10.4%)存在ANA阳性,与非IgA肾病慢性肾炎中的阳性率比较差异无统计学意义(P>0.05),ANA阳性的IgA肾病血浆IgG水平较高,病理改变较轻。结论 IgA肾病ANA阳性的机体免疫有增强,肾功、尿蛋白及病理损害较轻,其对IgA肾病预后的影响仍需进一步研究。Objective To describe the incidence of positive antinuclear antibody （ANA） in Immunoglobulin ANephropathy （IgAN） patients and its clinical significance. Methods We retrospectively reviewed the clinical data of 115patients with biopsy-proved IgAN who underwent ANA detection and 59 non-IgAN chronic glomerulonephritis patientsbetween January 2007 and December 2009. Then, we analyzed and compared the clinical and pathological features betweenpatients with positive and negative ANA. Results Twelve IgAN patients had positive ANA with a positive rate of 10.4%,which had no significant difference from the positive ANA rate in patients with non-IgAN chronic glomerulonephritis（P〉0.05）. Pathological changes and renal function in IgAN patients with positive ANA were less serious, and their levelof serum IgG was higher. Conclusion Positive ANA can promote immunity and lessen pathological changes in IgANpatients. However, its effect on the prognosis still needs further research.
