腰椎退变性滑脱合并椎间盘突出的诊断及手术治疗分析

摘　　要：目的探讨腰椎退变性滑脱合并椎间盘突出的影像学、临床特点及手术治疗效果。方法回顾分析89例腰椎退变性滑脱合并椎间盘突出患者的临床症状、体征及影像学检查,进行合理的手术评估,行后路次全椎板切除椎管减压、椎间盘髓核摘除、椎间隙植骨融合、椎弓根钉棒系统内固定术。结果手术治疗后优73例,良10例,可6例,优良率93.3%。术后随访1～3年,滑脱椎体复位,椎弓根钉棒系统内固定在位有效,无松动,临床症状基本消失。植骨融合率第一年为90.8%,第二年全部骨性融合。结论采用后路次全椎板切除椎管减压、椎间盘髓核摘除、椎间隙植骨融合、椎弓根钉棒系统内固定术,在处理腰椎间盘突出的同时纠正了腰椎滑脱,达到了脊柱稳定、解除症状的目的。Objective To investigatd the lumbar degenerative spondylolisthesis and LDH disc imaging,clinical features and surgical treatment.Methoda Aretrospective analysis of 89 cases of lumbar degenerative spondylolisthesis combined lumbar disc herniation in patients with clinical symptoms,signs and imaging studies,surgery assessment,posterior decompression laminecotomy,discectomy,the intervertebral space interbody fusion and pedicle screw system fixation.Results The surgical treatment were excellent in 73 cases,good in 10 cases,fair 6 cases,good rate 93.3%.The patients were followed up for1 to 3 years,spodylolisthesis reduction,pedicle screw system fixed reign,no loosening,clinical symptoms disappeared.90.8% fusion rate 1 year 2 years all bony fusion.Conclusion The posterior laminectomy spinal decompression,discectomy,and fusion of the intervertebral space,pedicle screw-rod system fixation,corrected handling of lumbar disc herniation lumbar spondylolisthesis,reaching spinal stability,and the purpose to relieve symptoms.
