国产和进口氨氟醚对肝肾功能影响的对比研究

摘　　要：目的　比较国产和进口氨氟醚对全身麻醉患者的肝肾功能影响。方法　选择 40例择期手术患者 ,ASAⅠ～Ⅱ级 ,术前检查肝肾功能正常。随机均分 2组 ,Ⅰ组 (2 0例 )术中吸入国产氨氟醚 ,Ⅱ组 (2 0例 )术中吸入进口氨氟醚。监测收缩压 (SBP)、舒张压 (DBP)、心率 (HR)、血氧饱和度(SPO2 )、心电图 (EKG)至手术结束。分别于麻醉诱导前、术中吸入氨氟醚 6 0min后、术毕停吸氨氟醚 30min后 ,抽取静脉血 2ml,检验肝肾功能 ,同时记录SBP、DBP、HR、SPO2 值。结果　两组患者术前SBP、DBP、HR、SPO2 及肝肾功能均正常 ,两组之间差异无显著性。两组患者麻醉效果相近 ,国产和进口氨氟醚的麻醉强度相似。吸入氨氟醚 6 0min后、术毕停吸氨氟醚 30min后 ,两组患者SBP下降 ,HR均上升 ,总蛋白、白蛋白、球蛋白、前蛋白均明显下降 ,肌酐、尿酸也有下降趋势 ;肝肾功能的 16项指标组间差异无显著性 (P >0 0 5 )。结论　国产氨氟醚和进口氨氟醚一样对肝肾功能、血流动力学无明显影响 ,国产氨氟醚麻醉效能不亚于进口氨氟醚 。Objective To compare the effect of inhalation of domestic and imported enflurane on hepatic and renal functions of the general anesthesia patients. Methods 40 operative patients, ASA grade Ⅰ～Ⅱ. Their hepatic and renal functions were normal in preoperation. 40 patients were randomly divided into 2 groups, 20 patients a group. The patients in GroupⅠ were inhalated by the home made enflurane during operation. The patients in GroupⅡ were inhalated by the imported enflurane during operation. The BP, HR, SPO 2, EKG were monitored with NBP 4000 Monitor . 2ml vein blood was taken before anesthesia, after 60 min inhalating enflurane, after 30 min of stopping inhalating enflurane, respectively, for examining hepatic and renal functions. Results Preoperative BP, HR, SPO 2 hepatic and renal functions did not show significant difference between 2 groups, respectively. TPR, ALB, GLB and PA were significantly lower in 2 groups after 60 min inhalating enflurane, after 30 min of stopping inhalating enflurane, respectively, but 16 monitoring parameters compared between 2 groups did not reveal significant difference. Conclusion The effect of home made and imported enflurane on hepatic and renal functions and hemodynamics are not significantly different. Therefore, the domestic enflurane is an ideal inhalation anesthetic.
