彩超评价甲状腺动脉栓塞治疗甲亢前后外周动脉的变化

摘　　要：目的探讨彩色多普勒超声评价甲状腺动脉栓塞治疗甲亢后外周动脉变化的价值。方法43例甲亢患者行甲状腺动脉栓塞治疗，于术前及术后1、3个月用彩色多普勒超声观察测量颈总动脉（CCA）、颈内动脉（ICA）、颈外动脉（ECA）、肾动脉（RA）搏动指数（PI）与阻力指数（RI）以及颈动脉内一中膜厚度（IMT）。结果甲亢组治疗前外周动脉搏动指数、ICA和RA阻力指数以及颈动脉内一中膜厚度明显高于对照组（P〈0．05，0．01），ECA阻力指数明显低于对照组（P〈0．05）。治疗3个月后，搏动指数、ICA和RA阻力指数和颈动脉内一中膜厚度明显低于治疗前（P〈0．05～0．01），CCA阻力指数与对照组及治疗前比较，无统计学差异（P〉0．05）。结论彩色多普勒超声对甲状腺动脉栓塞治疗甲亢的疗效评价具有重要价值。Objective To explore the value of assessing the change of peripheral arteries after thyroid-artery-embolism treatment on hyperthyroidism with colour doppler ultrasonography. Methods 43 hyperthyroidism patients were treat with thyroid artery embolism; resistent index （ RI ）, pulsatility index （ PI ） of common carotid artery （ CCA ）, internal carotid artery （ ICA ）, external carotid artery （ ECA ）, renal artery（ RA ）, and carotid artery intima-media thicknes（ IMT ） were measured before and 1, 3 months after the operation with colour doppler uhrasonography. Results pulsatility index, resis- tance index of ICA and RA and IMT of carotid artery of the hyperthyroid group before treatment was significantly higher than that of control group （ P〈 0.05 ）, while resistance index of ECA was significantly lower than that of the control group （ P〈 0.05 ）. 3 months after the treatment, pulsatility index, resistance index of 1CA and RA and 1MT of carotid artery of ICA and RA was significantly lower than that before treatment （ P 〈 0.05 ）; resistance index of CCA wasn＇t statistically different between the hyperthyroid group and the control group and didn＇ t significantly differ from that before the treatment （ P 〉 0.05 ）. Conclusion color Doppler ultrasound has important value for the clinical-efficacy assessment of thy- roid-artery-embolism treatment on hyperthyroidism.
