摘  要:目的:探讨老年男性骨质疏松患者血清瘦素(Leptin)水平的变化及其与骨密度(BMD)及骨转换生化指标的关系。方法:采用酶联免疫法(ELISA)测定89例老年男性骨质疏松患者和50例正常男性老年人血清Leptin、骨碱性磷酸酶(BAP)、骨钙素(BGP)、抗酒石酸酸性磷酸酶-5b(TRACP-5b),采用美国NORLAND Excell-plus双能X线骨密度测定仪分别测定其正位腰椎(L2~4)及左侧股骨颈处BMD。结果:老年男性骨质疏松患者正位腰椎及左侧股骨颈BMD、血清Leptin、BAP、BGP水平较正常男性老年人明显降低(均P〈0.01),血清TRACP-5b水平较正常男性老年人明显升高(P〈0.01)。老年男性骨质疏松患者血清leptin水平与患者正位腰椎及左侧股骨颈BMD、血清BAP、BGP水平呈明显正相关(r=0.437,r=0.496,r=0.512,r=0.489,均P〈0.01),与患者血清TRACP-5b水平呈明显负相关(r=-0.529,P〈0.01)。结论:血清瘦素水平降低在老年男性骨质疏松的发病中可能发挥重要作用。Objective:To investigate the changes in serum leptin and the correlation between serum leptin and bone minaral dencity(BMD) and bone turnover biocamical markers.Methods:The serum leptin,bone alkaline phosphatase(BAP),bone gla protein(BGP) and tartrate-resistant acid phosphate(TRACP-5b) were measured by using ELISA,the BMD of the lumber vertebrae 2~4(L2~4) and the neck of femur(Neck) were measured with dual energy X-ray absorptionetry in 89 aged males with osteoprosis and 50 normal aged males.Results:BMD of the lumber vertebrae 2-4 and the neck of femur,serum leptin,BAP and BGP were significantly lower(all P0.01),serum TRACP-5b was significantly higher(P0.01) in the aged males with osteoprosis than those in normall aged males.A positive correlation was found between serum leptin and BMD of the lumber vertebrae 2-4 and the neck of femur and serum BAP and BGP(r=0.437,r=0.496,r=0.512,r=0.489,all P0.01),a negative correlation was found between serum leptin and serum TRACP-5b(r=-0.529,P0.01).Conclusion:The low serum concentration of leptin might play an important role in the development of osteoprosis in the aged male.
