无创呼吸机治疗AECOPD合并Ⅱ型呼吸衰竭临床疗效回顾性分析

摘　　要：目的探讨无创正压通气治疗慢性阻塞性肺疾病急性加重期(AECOPD)合并Ⅱ型呼吸衰竭患者临床疗效。方法选取202例AECOPD合并呼吸衰竭的患者接受无创正压呼吸机的治疗,分为有合并症组A组(合并高血压、冠心病、糖尿病)和无合并症组B组,分别比较两组治疗前后动脉氧分压(PaO2)、动脉血二氧化碳分压(PaCO2),比较两组撤机时间的统计学差异性。结果与A组相比较,撤机时间无差异(P>0.05),治疗后PaO2、PaCO2无差异(P>0.05);各组无创正压通气治疗前后PaO2、PaCO2差异有统计学意义(P<0.05)。结论早期合理利用无创正压通气,可明显改善AECOPD呼吸衰竭患者PaO2、PaCO2;合并症并无降低其治疗效果和延长撤机时间。Objective To investigte the clinical effect of noninvasive positive pressure ventilator（NIPPV） in patients with acute exacerbations of chronic obstructive pulmonary disease and type Ⅱ respiratory failure.Methods Two hundred and two AECOPD patients treated NIPPV were divided into group A and B according to the complications（Hypertention,coronary heart disease,and diabetes mellitus）,each group before and after therapy of PaO2,PaCO2 were analyzed comparatively;the times of NIPPV were also compared between two groups.Results Compared to group A,the time and PaO2,PaCO2 after the treatment had no difference（P0.05）;Compared to PaO2,PaCO2 before treatment,each group＇s PaO2,PaCO2 had differences（P0.05）.ConclusionsEarly and reasonable using of NIPPV could obviously improve the PaO2 、PaCO2 in patients with acute exacerbations of chronic obstructive pulmonary disease and type Ⅱ respiratory failure,and the complications did not step down the clinical effect and lengthen the using time.
