加巴喷丁复合吗啡预防慢性压迫坐骨神经致痛觉敏化的疗效分析

摘　　要：目的观察加巴喷丁复合吗啡预防性镇痛对大鼠慢性压迫坐骨神经(CCI)后导致的机械缩足阈变化以及脊髓背角肿瘤坏死因子-α(TNF-α)表达的影响。方法随机选择重量(180±20)g的成年雄性SD大鼠24只,分为假手术组(S组)、模型组(M组)、预防性镇痛组(P组)和常规镇痛组(N组)。观察不同方式给药的模型组和对照组大鼠的行为学变化。术后10d分离脊髓通过免疫组化检测TNF-α水平的变化。结果术前各组大鼠机械痛阈差异无统计学意义,与M组相比,手术后各时段S、P、N组都存在不同程度的改变,差异均有统计学意义(P<0.05);P、N组TNF-α的积分光密度均值(IOD)水平分别为(0.2185±0.01980)、(0.2301±0.01386),与M组的(0.2902±0.01325)比较明显降低,差异均有统计学意义(P<0.05)。结论加巴喷丁复合吗啡对大鼠采用预防性镇痛有较好的效果,可显著降低CCI后导致的机械痛阈,降低TNF-α在脊髓背角的表达。Objective To investigate the effect of Gabapentin combined with Morphine in preventive analgesia on mechanical withdrawal threshold and the expression of TNF-α expression of tumor necrosis factor-alpha in dorsal horn in sciatic nerve chronic constrictive injury （CCI） rats. Methods Twenty four male Sprague-Dawley（SD） rats were randomly divided into sham operation group（S）, model group（M）, preventive analgesia group（P） and normal analgesia group（N）. Mechanical pain threshold on the operated side of rats in different groups were measured. Rats were killed on the 10th day after operation..The dynamic changes of TNF-α in spinal dorsal corn neuron were detected by immunohistochemical method. Results The depression of mechanical pain threshold was more responsive in M than P and N after the 10h day of the surgery. The expression of TNF-α in the superficial spinal dorsal horn on the operated side of rats in M was higher than those in P and N（P0.05）. Conclusion Preemptive administration of Gabapentin combined with Morphine which have good effect, can significantly decrease severity of mechanical hyperalgesia induced by CCI, as well as the expression of TNF-α.
