载脂蛋白B与A-1比值对预测血管疾病风险的价值

摘　　要：目的评价、比较载脂蛋白B与A-1比值(apoB/apoA-1)和其他5种血脂指数在心脑血管疾病风险预测中的价值,探讨apoB/apoA-1比值警示界线的划分。方法分析测量3 972例受试者血脂水平,计算:血浆致动脉硬化指数、动脉硬化指数以及低密度脂蛋白、胆固醇、三酰甘油分别与高密度脂蛋白的比值和apoB/apoA-1比值;分别依年龄、性别、apoB/apoA-1阈界进行分组比较。结果同性别不同年龄段apoB/apoA-1比值比较,>60岁组女性最高(P<0.01),40～60岁组男性最高(P<0.01);年龄>60岁者,apoB/apoA-1在阈界0.9时男、女阳性率分别为20.7%,25.8%;不同性别6项指数比较,男性apoB/apoA-1明显高于女性(P<0.01);以apoB/apoA-1阈界0.9分组比较各项血脂指数,apoB/apoA-1差异最大(F=2 909.01)。结论在预测心脑血管疾病上apoB/apoA-1比值优于其他血脂指数,是预测心脑血管疾病风险敏感、准确的指数,apoB/apoA-1≥0.9为警示界线。Objective To evaluate and compare the value of apoB/apoA-1 ratio and other five blood lipid indexes to the predicting cardio-cerebrovascular disease for investigate warning line of apoB/apoA-1 ratio.Methods A total of 3 972 cases were detected six blood lipid indexes to calculate atherogenic index of plasma,ratios of low density lipoprotein,cholesterol and triacylglycerol to high density lipoprotein,atherosclerosis index and apoB/apoA-1 ratio,and were compared according to the age,sex,and threshold value.Results ApoB/apoA-1 ratio was the highest in those aged over 60 years（P0.01） in the female patients,and highest in those aged from 40 to 60 years（P0.01） in the male patients,when they were compared in the same sex group.The positive rate of apoB/apoA-1 was 20.7% and 25.8% in males and females when the warning line was 0.9.The apoB/apoA-1 ratio was obviously higher in males than that in females in these 6 indexes.And when apoB/apoA-1 warning line was 0.9,apoB/apoA-1 showed the biggest difference in all indexes（F=2909.01）.Conclusion The apoB/apoA-1 ratio is superior to the other blood lipid indexes for predicting cardio-cerebrovascular diseases.It is a sensitive and accurate index of cardio-cerebrovascular disease.The warning line of apoB/apoA-1 is 0.9 or bigger.
