责任血管微血管减压术治疗面肌痉挛延迟治愈69例临床分析

摘　　要：目的分析不同类型责任血管微血管减压手术(MVD)治疗面肌痉挛术后延迟治愈的发生情况。方法依据责任血管的不同将获得随访2年以上的230例MVD术患者分为小血管压迫(A组,164例)和椎-基底动脉复合体压迫(B组,66例)两组,分析发生延迟治愈的69例随访资料。结果 230例面肌痉挛行MVD术患者中,术后延迟治愈69例(30.0%)。A组延迟治愈率26.2%(43/164),明显低于B组的39.4%(26/66)(P<0.05)。69例延迟治愈患者中,术后1、2、3、6、12个月和1年及以上症状消失病例分别为33、13、12、7、3和1例。94.8%的延迟治愈患者术后半年抽搐停止。结论面肌痉挛患者MVD术后延迟治愈以椎-基底动脉复合体压迫致病者居多;大部分延迟治愈患者痉挛症状可于术后6个月内消失。Objective To analyze the postoperative delay cure in the patients with hemifacial spasm after microvacular decompression of the different distinct offending vessels.Methods A total of 230 patients underwent microvacular decompression surgery for hemifacial spasm was successfully followed up for more than 2 years.Based on the distinct offending vessel,the patients were divided into two groups of A（164 cases,received small vascular compression） and B（66 cases,received vertebral-basal artery complex compression）.Data of 69 cases with delay were analyzed.Results Of 230 cases,69 cases（32.7%） were delay cured.The delay cure rate was lower in group A than that in group B [26.2%（43/164） vs.39.4%（26/66）]（P〈0.05）.Of 69 cases with delay cure,the symptoms of 33 cases disappeared in one month after operation,13 cases in two months,12 cases in three months,7 cases in six months,3 cases in one year,and 1 case more than one year.Hemifacial spasm in 94.8% delay cured patients disappeared in six months after operation.Conclusion The postoperative delay cure in the patients with hemifacial spasm after MVD occures mostly in the cases whose hemifacial spasm is induced by vertebral-basal artery complex compression.The symptoms in majority of them disappeare within six months after MVD.
