血清脂蛋白相关磷脂酶A2和超敏C-反应蛋白水平与大动脉粥样硬化性脑梗死关系的临床研究

摘　　要：目的探讨脂蛋白相关磷脂酶A2(Lp-PLA2)、超敏C-反应蛋白(hs-CRP)与大动脉粥样硬化性脑梗死的关系。方法选取33例大动脉粥样硬化性脑梗死患者为病例组,同时选择年龄性别相匹配的健康体检者30例为对照组;酶联免疫吸附法(ELISA)测定血清Lp-PLA2,采用全自动生化仪测定血清hs-CRP水平,常规生化法检测血脂指标、尿酸(UA)、纤维蛋白原(FIB)。结果大动脉粥样硬化性脑梗死组Lp-PLA2、hs-CRP、LDL-C、TC、TG、UA、FIB水平、高血压发生率较对照组增高,差异有统计学意义(P<0.05);Lp-PLA2与HDL-C呈负相关(r=-0.104,P=0.027),与TC、LDL-C、脂蛋白(a)呈正相关(r=0.162,P=0.024;r=0.083,P=0.015;r=0.412,P=0.017)。结论血清Lp-PLA2、hs-CRP参与大动脉粥样硬化性脑梗死的病理生理过程。Objective To investigate the relationship between plasma lipoprotein associated phospholipase A2（Lp-PLA2）,high-sensitivity C-reactive protein（hs-CRP）and large artery atherosclerosis ischemic stroke.Methods Thirty-three patients with large artery atherosclerosis ischemic stroke were assigned as experimental group and thirty healthy people as control group.The levels of Lp-PLA2,hs-CRP,blood fat,uric acid（UA）,fibrinogen（FIB）were measured.Results The levels of serum Lp-PLA2,hs-CRP,low density lipoprotein-cholesterol（LDL-C）,TC,TG,UA,FIB,the incidence of hypertension were significantly higher in the experimental group compared with the control（P0.05）.The plasma level of Lp-PLA2 was negatively correlated with HDL-C（r=-0.104,P=0.027）,positively corelated with TC,LDL-C and Lipoprotein-a（r=0.162,P=0.024;r=0.083,P=0.015;r=0.412,P=0.017）.Conclusions Lp-PLA2 and hs-CRP participate in the pathophysiology of acute cerebral infarction process.
