摘  要:目的探讨延肾口服液对肾间质纤维化(RIF)大鼠的影响及可能的作用机制。方法将75只SD雄性大鼠按随机数字表法分为假手术组和模型组,采用单侧输尿管梗阻(UUO)复制RIF模型,成模大鼠再随机分为模型组、贝那普利组及延肾口服液高、低剂量组,每组15只。术前1d治疗组开始灌胃贝那普利3.5ml/d及延肾口服液2.8ml/d、1.4ml/d;假手术组、模型组给予等量生理盐水。术后8周留取血、尿标本,行血肌酐(SCr)、血尿素氮(BUN)及尿肌酐(ucr)、尿尿素氮(UUN)和24h尿蛋白定量测定;处死大鼠取。肾组织,苏木素-伊红(HE)染色观察各组RIF程度;用免疫组化二步法和半定量逆转录-聚合酶链反应(RT-PCR)测定转化生长因子-β(TGF—β)、α-平滑肌肌动蛋白(α-SMA)的蛋白和mRNA表达。结果①光镜下观察,模型组肾管上皮细胞呈现不同程度的萎缩、变性及坏死,小管周围肾间质间隙增宽;贝那普利组和延肾口服液高、低剂量组部分肾小管轻度扩张,病变程度轻于模型组。②与假手术组比较,模型组UUN、UCr、BUN及TGF-β、α-SMA的蛋白和mRNA表达均明显升高(均P〈0.01);与模型组比较,贝那普利组及延肾口服液高、低剂量组TGF-β、α—SMA蛋白和mRNA[吸光度(A)值]表达均显著减少,以延肾口服液高剂量组减少更显著(TGF-β蛋白:0.72±0.24比1.78±0.33,α-SMA蛋白:0.87±0.55比1.59±0.44;TGF—BmRNA:1.68±0.53比3.14±0.44,α-SMAmRNA:1.66±0.14比3.41±0.10,均P〈O.01);贝那普利组和延肾口服液高、低剂量组UUN(mmol/L)、UCr(p.mol/L)、BUN(mmol/L)均明显降低(P〈0.05或P〈O.01),以延肾口服液高剂量组降低更显著(UUN:0.56±0.19比1.34±0.22,UCr:2788.00±178.98比3652.75±188.26,BUN:7.25±0.47比16.30±0.47,均P〈0.01)�Objective To investigate the influence and its mechanism of Yanshen oral liquid (YSH) on rats with renal interstitial fibrosis (RIF). Methods Seventy-five Sprague-Dawley (SD) male rats were randomly divided into sham operation group and model group. The rat RIF model was reproduced by unilateral ureteral obstruction (UUO), and the model rats were randomly divided into model, benazepril and high-YSH treatment, low- YSH treatment groups (each n=15). One day before the operation, the treatment began, benazepril 3.5 ml/d, YSH 2.8 ml/d and 1.4 ml/d were respectively infused into the stomach in benazepril, high- and low-YSH treatment groups, and in sham operation group and model group, the same amount of physiological saline was given. After treatment for 8 weeks, the concentrations of serum ereatinine (SCr), blood urea nitrogen (BUN), urine creatinine (UCr), urine urea nitrogen (UUN) and 24-hour urinary protein were detected. The dynamic histological changes of RIF were observed by hematoxylin and eosin (HE) staining. The protein and mRNA expressions of transforming growth factor- β (TGF-β)and oL-smooth muscle aetin (α-SMA) were detected by immunohistochemistry and reverse transcription- polymerase chain reaction (RT-PCR). Results (1) Under the light microscope, the tubular cells of model group exhibited various degrees of degeneration, atrophy and necrosis, and the interstitial tissue around the tubule became broadened ; in benazepril group, high-YSH and low-YSH treatment groups, there were part of the renal tubules mildly dilated, the lesion being less serious than that in the model group. (2) Compared with the sham operation group, UUN, UCr, BUN and TGF-β, α-SMA protein and mRNA expressions were markedly higher in the model group (all P〈 0.01). Compared with the model group, TGF-β, α-SMA protein and mRNA [ absorbance (A) value] expressions in benazepril group, high-YSH and low-YSH treatment groups were significantly reduced, and the extent
