经阴道超声评估宫颈形态与早产关系的临床研究

摘　　要：目的:探讨通过超声识别宫颈形态与早产的关系。方法:前瞻性纳入2010年6月至2011年4月期间于我院建卡定期产前检查的孕妇150例,并根据超声检查结果和纳入标准分为3组,即U形漏斗宫颈组,V形漏斗宫颈组,正常宫颈形态组。比较3组之间的先兆早产率、早产率和早产时间的差异。结果:本研究实际纳入病例140例,其中宫颈U形漏斗组47例,V形漏斗组56例,宫颈形态正常组37例。U形漏斗宫颈的早产率明显高于V形漏斗和宫颈形态正常的孕妇,差异均有统计学意义(P<0.05),V形漏斗和正常宫颈形态的孕妇早产率差异无统计学意义;并且U形漏斗宫颈的分娩孕周明显早于V形漏斗宫颈和正常宫颈形态的孕妇(P<0.05)。结论:孕妇的宫颈形态对于早产有一定的影响,因此应对有高危因素的孕妇筛查宫颈形态,以针对性地预防早产。Objective ：To investigate the relationship between cervical shape identified by ultrasound and preterm labor. Methoda：150 pregnant women examed regularly in our hospital from June 2010 to April 2011 were prospectively enrolled. All of them were equally assigned into three groups according to inclusion crite- ria and ultrasound results such as u-shaped funnel cervical group, v-shaped funnel cervical group, and the normal cervical group. The threatened premature labor rate, preterm delivery rate and preterm delivery time were compared among the three groups. Results： 140 patients were finally included, with 47 patients of u-shaped funnel cervical, 56 patients of v-shaped funnel group and 37 cases of normal cervical form. The preterm labor rate in u-shaped funnel cervical group was significantly higher than that in v-shaped funnel group and normal cervical group. There was significant statistical difference （ P 〈 O. 05）. No obvious differ- ence was detected in preterm labor rate between v-shaped funnel and normal cervical group. Moreover, the delivery occurred significantly earlier in u-shaped cervical funnel group as compared to v-shaped and normal cervical form（ P〈O. 05）. Conclusions.The shape of cervix in pregnant women has some influence on preterm labor, which suggests that cervical shape should be scanned for high-risk women to pertinently prevent preterm delivery.
