摘　　要：目的 探讨利用双叶瓣法旋转修复局限性鼻部软组织缺损的美容效果,并总结手术操作的技巧和经验.方法 根据缺损的位置、形状和面积,设计邻近的双叶皮瓣,缺损纵轴和第二叶皮瓣纵轴呈90°～100°,皮瓣面积依次减小,通过＂递接＂式转移,最终完全覆盖创面及皮瓣供区.结果 16例患者获随访4～14个月,除鼻翼缘缺损的患者中有1例轻度鼻翼缘退缩,1例轻度鼻孔形状改变;所有缺损均得到很好的修复,鼻亚单位保存完好,无明显牵拉和扭曲,形态协调、自然.结论 对局限性鼻部软组织缺损,双叶瓣法是个很好的选择,可获得满意的手术效果,但是要选择好手术适应证,并严格遵循皮瓣的设计和操作原则.Objective To investigate the aesthetic results of limited nasal soft tissue defects with bilobed flap, and summarize the surgical techniques and experiences. Methods The adjacent bilobed flap was designed and performed according to the defect location, shape and size. The included angle between the vertical axises of the defect and the second flap was about 90° to 100°. The size of the second flap was smaller than the first one, and then the defect and donor site were well covered by this successive flap migration. The follow-up results were acquired and evaluated. Results After 4 to 14 months follow-up, all 16 cases were satisfied with beautiful and natural appearance and without obvious distort and misshape except for one case with slight retraction of nasal ala edge and one case with slight deformity of nostril. Conclusion The bilobed flap is an ideal method for reconstructing the limited defects of nasal soft tissue with good color, contour and texture under following the design principles and surgical techniques.
