Maspin、bFGF在宫颈病变中的表达及与高危型HPV的相关性研究

摘　　要：目的:通过检测乳腺丝氨酸蛋白酶抑制剂(Maspin)、碱性成纤维细胞生长因子(bFGF)在慢性宫颈炎、宫颈上皮内瘤样变(CIN)及宫颈癌组织中的表达,探讨两者的相关性及与高危型人乳头状瘤病毒(高危型HPV)感染之间的关系。方法:宫颈石蜡组织标本166例,其中高危型HPV(+)组106例(慢性宫颈炎20例、CINⅠ级20例、CINⅡ级20例、CINⅢ级20例、宫颈癌组26例);HPV(-)组60例(CINⅠ级35例、CINⅡ级20例、CINⅢ级5例)。应用SP法检测Maspin、bFGF的表达情况。结果:(1)在高危型HPV(+)组中,Maspin蛋白的阳性表达率呈逐渐下降趋势(P<0.05),且在CINⅢ级组中的表达同慢性宫颈炎组、CINⅠ级组、CINⅡ级组相比,差异均有统计学意义(均P<0.05),在宫颈癌组中的阳性表达与CIN各组、慢性宫颈炎组相比较,差异均有统计学意义(均P<0.05);bFGF的阳性表达率呈逐渐增加趋势(P<0.05),bFGF阳性表达率在宫颈癌组表达率最高,显著高于慢性宫颈炎组、CINⅠ级组、CINⅡ级组,差异均有统计学意义(均P<0.05)。在HPV(-)组中,Maspin蛋白的表达依次降低,各组间比较差异均无统计学意义(P>0.05);bFGF的表达依次升高,但差异无统计学意义(P>0.05)。(2)Maspin、bFGF蛋白的表达呈负相关,两者与高危型HPV分别呈负、正相关。结论:高危型HPV可能通过P53调节通路下调Maspin的蛋白表达及上调bFGF的蛋白表达,导致CIN直至宫颈癌的发生。Objective： To detect the expression of mammary serpin（Maspin）,basic fibroblast growth factor（bFGF） in patients with chronic cervicitis,cervical intraepithelial neoplasia and cervical cancer and explore the relationship between Maspin,bFGF and high-risk human papilloma virus（high-risk type HPV） infection.Methods： Cervical paraffin tissue samples were collected（n=166） in which 106 cases were in high-risk type HPV positive group（chronic cervicitis 20 cases,CINⅠgrade 20 cases,CINⅡ grade 20 cases,CINⅢgrade 20 cases,cervical cancer 26 cases）;60 cases were in HPV negative group（CINⅠgrade 35 cases,CINⅡgrade 20 cases,CINⅢ grade 5 cases）.Expression of Maspin,bFGF protein was detected by Immunohistochemical SP method.Results：（1）High-risk type HPV（＋） group： The rate of Maspin protein positive expression was gradually decreased（P 0.05）,The level of Maspin protein positive expression in CIN Ⅲ was statistically lower than those of chronic cervicitis,CINⅠgrade group,and CIN Ⅱ grade group（P 0.05）,The rate of Maspin protein positive expression in cervical cancer was significantlly lower than those in each group of CIN,chronic cervicitis group（P 0.05）;The positive expression rate of bFGF was significantly increasing（P 0.05）,The positive rate of bFGF expression in the cervical cancer was highest,It was significantly higher than the rate of chronic cervicitis,CINⅠgrade group,CINⅡgrade group（P 0.05）.HPV（-） group of CIN ：The positive rates of Maspin protein were gradually lower among CIN groups but the difference between groups was not statistically signif icant（P0.05）;The positive rate of bFGF in CIN groups was gradually increased but the difference was not statistically significant（P0.05）.（2）The relationship of Maspin protein expression and bFGF protein expression was negatively correlated.the relationship of Maspin protein expression and high-risk type HPV infection was negatively correlated and the relationship of bFGF protein expr
