摘 要:目的观察降糖三黄片对糖尿病肾病(DN)大鼠肾组织中蛋白激酶C(PKC)和转化生长因子β1(TGFβ1)的影响及其对肾脏的保护作用。方法将49只Wistar大鼠随机分为正常组10只,造模组39只。采用高脂饲料加链脲佐菌素腹腔注射复制DN大鼠模型,34只造模成功大鼠分为模型组10只,中药组和西药组各12只。中药组给予降糖三黄片[787.5mg/(kg.d)],西药组给予卡托普利片[4mg/(kg.d)],正常组及模型组灌服等量蒸馏水,各组均连续灌胃8周。实验末腹主动脉采血检测各组大鼠血糖、血脂、胆固醇和胰岛素水平;治疗第4、8周测24h尿蛋白总量;摘取右侧肾脏称重并计算肾质量系数;免疫组织化学检测肾组织TGF-β1水平和PKC活性。结果中药组血糖、血脂、胰岛素水平下降,胰岛素敏感指数上升,与模型组及西药组比较差异有统计学意义(P<0.01或P<0.05)。中药组肾质量系数低于模型组(P<0.05),中药组和西药组尿蛋白总量均较模型组减少(P<0.01);中药组肾质量系数、尿蛋白总量与西药组比较差异均无统计学意义。中药组、西药组大鼠TGF-β1、PKC表达明显低于模型组(P<0.01),西药组略低于中药组但两组比较差异无统计学意义。结论降糖三黄片可抑制DN大鼠肾组织中PKC活性,下调TGF-β1水平,使细胞外基质合成减少,起到保护肾组织、推迟肾纤维化的作用。ABSTRACT Objective To observe the effect of Jiangtang Sanhuang Tablet (Tablet to reduce blood sugar) on protein kinase C (PKC) and transforming growth factor-β1 (TGF-β1) in the nephridial tissues of rats with diabetic nephropathy (DN) and its protection for the kidney. Methods Forty-nine Wistar rats were randomized into normal group (n= 10), model group (n= 39). DN models were established with high fat diet and intraperitoneal injection of streptozotocin. Totally 34 successful model were divided into model control group (n= 10), Chinese medication group (n= 12) and western medication group (n= 12). Chinese medication group was given 787.5mg/(kg · d) of Jiangtang Sanhuang Tablet and western medication Rrouo was Riven4mg/(kg · d) of Captopril Tablet through gastric feeding. The normal group and model control group were given normal sa- line. All groups were treated for 8 weeks. At the end of the experiment, the levels of blood glucose, blood fat, cholesterol, in- sulin were determined, and 24h urinary protein (Upro) were detected on the 4th and 8th week. The right kidney was selected and ratio of kidney weight to body weight was evaluated. The level of TGF-β1 and the activity of PKC in the nephridial tissues were detected by immunohisto-chemical method. Results Comparing with the model control group, the levels of blood glu-cose, blood fat and insulin decreased significantly, but the insulin sensitivity index (ISI) increased significantly in Chinese medi- cation group (P〈0.01 or P〈0.05). The ratio of kidney weight to body weight in Chinese medication group was lower than that in the model control group (P〈0.05). The total urinary protein (TUP) in Chinese medication group and western group was lower than that in the model group (P〈0. 01).
