人工股骨头置换术治疗老年股骨粗隆间骨折疗效观察

摘　　要：目的探讨人工股骨头置换术及动力髋螺钉(DHS)内固定术治疗老年股骨粗隆间骨折(IFF)的疗效及远期并发症情况。方法回顾性分析行DHS内固定术及人工股骨头置换术的老年IFF患者69例的临床资料,比较两组年龄、骨折分型、手术时间、术中出血量、术后负重时间、住院时间,以及随访1、6个月的Harris评分。结果人工股骨头置换组术后1个月Harris评分高于DHS内固定组(P<0.01),术后可负重时间短于DHS内固定组(P<0.01)。结论人工股骨头置换术适用于高龄股骨粗隆间骨折并骨质疏松的患者,可使其术后早期下床活动,并发症减少。Objective To compare hip hemiarthroplasty with internal fixation for treatment of intertrochanteric fracture （IFF） in the elderly.Methods The data of 69 cases of IIF managed with hip hemiarthroplasty（observation group,n=33） or internal fixation（control group,n=36） were retrospectively analyzed,comparing their fracture type according to Tronz-Evans classification,operative time,blood loss,hospitalization time,the loading time, postoperative complications and Harris scores of 1 and 6 months.Results The Harris score of observation group was statistically higher than the control 1 month postoperatively（P0.01）. The postoperative weight bearing time of the observation shortened as compared with the control（P0.01）.Conculsion Hemiarthroplasty is the reasonable choice in treating IFF in the elderly.
