带旋股外侧血管升支髂骨瓣与横支大转子骨瓣治疗成人股骨头坏死疗效对比分析

摘　　要：目的比较采用两种不同的显微修复方法治疗成人股骨头坏死的疗效与优缺点。方法随机选取2001年1月1日～2010年12月30日,分别采用带旋股外侧血管升支髂骨瓣和横支大转子骨瓣治疗的股骨头坏死FicatⅢ期的患者各100例行追踪复查。结果经统计学分析带旋股外侧血管升支髂骨瓣与横支大转子骨瓣相比Harris评分的改善及股骨头塌陷的高度差异有统计学意义,但升支髂骨瓣平均切口比横支大转子骨瓣长5.2cm,且升支髂骨瓣组患者有3例股外侧皮神经损伤症状。结论两种方法治疗成人股骨头坏死的疗效都很确切,但带旋股外侧血管升支髂骨瓣与横支大转子骨瓣相比疗效更好。而横支大转子骨瓣具有切口更小且没有股外侧皮神经损伤风险的优势。Objective To compare using two different reconstruction methods in the treatment of osteonecrosis of femoral head（ONFH） of the efficacy and advantages and disadvantages. Methods Randomly choose between January 1, 2001 and December 30, 2010, respectively, with ascending branch of lateral circumflex femoral artery iliac bone flap and the transverse branch of greater trochanter bone flap for the treatment of ONFH （Ficat III） with 100 routine follow-up review. Results The statistical analysis with ascending branch of lateral circumflex femoral artery iliac bone flap with the transverse branch of the great trochanter bone flap compared with Harris score improvement and collapse of femoral head height difference is statistically significant, but the ascending branch of iliac flap incision than average transverse branch of the great trochanter bone flap length 5.2cm, and the ascending branch of iliac flap group patients in 3 cases lateral femoral cutaneous nerve injury symptoms. Conclusion The two kinds of methods for the treatment of ONFH effect is exact, but the lateral femoral circumflex artery ascending iliac bone flap with the transverse branch of the great trochanter bone flap compared with better curative effect. While the transverse branch of the great trochanter bone flap with a smaller incision and no lateral femoral cutaneous nerve injury risk advantage.
