摘 要:目的:探讨双定位体外冲击波碎石机治疗泌尿系结石的临床疗效。方法:用X线和B超双定位体外冲击波碎石机治疗上尿路结石患者300例(实验组),并与同期用B超定位的体外冲击波碎石机治疗的上尿路结石患者300例(对照组)的疗效进行比较。结果:实验组手术时间、结石排净时间均显著短于对照组(P<0.05),输尿管上段、中下段结石定位成功率(分别为99%、98%)均明显高于对照组(分别为74%、68%),差异有统计学意义(χ2分别为4.35、6.32,P均<0.05),实验组在输尿管上段、中下段结石一次性碎石成功率(分别为92%、90%)均明显高于对照组(分别为65%、63%),差异有统计学意义(χ2分别为6.25、6.16,P均<0.05)。实验组术后感染3例,出现"石街"2例,对照组术后感染4例,出现"石街"3例,两组并发症发生率比较差异无统计学意义(P>0.05)。结论:X线和B超双定位系统体外冲击波碎石机在协助定位上取长补短,提高定位成功率和碎石成功率,缩短了手术时间,促进了患者康复。Objective: To explore the effect of double positioning extracorporeal shock wave lithotripsy on urinary calculi patients.Methods:A total of 300 patients with upper urinary tract calculi were treated by double orientation extracorporeal shock wave lithotripsy(experimental group),and another 300 upper urinary tract calculi patients were treated by B ultrasound localization extracorporeal shock wave lithotripsy at same period(control group).The therapeutical effect was compared between two group patients.Results:The operation time,lithagogue time of experimental group was significantly shorter than control group(P 0.05).The success rates of upper ureter,middle-lower ureteral calculi positioning in the experimental group were significantly higher than control group(χ2 = 4.35,χ2 = 6.32,P0.05).The success rates of lithotripsy in the upper ureter,middle-lower ureteral were significantly higher in experimental group than control group(χ2 = 6.25,χ2 = 6.16,P0.05).There were 3 cases with infection after surgery in experiment group,including 2 cases with steinstrasse,and 4 cases with infection in control group,including 3 cases with steinstrasse.But there was no significant difference in complication rate between two groups(P0.05).Conclusions: Double positioning system extracorporeal shock wave lithotripsy machine with X and B ultrasound assisted positioning can improve the success rate of positioning and the success rate of lithotripsy,shorten the operation time,and promote the rehabilitation of patients.
