摘  要:目的:探讨中性粒细胞表面标志物CD64、C-反应蛋白(CRP)、白细胞介素-6(IL-6)在儿科感染性疾病中的应用价值。方法:116例住院患儿分为3组,重症感染组49例,一般感染组36例,非感染组31例,对3组CD64、CRP、IL-6测定结果进行比较。结果:重症感染组、一般感染组CD64水平明显高于非感染组,重症感染组CD64水平明显高于一般感染组,3组间两两比较,差异有统计学意义(H=35.02,P<0.01);重症感染组与一般感染组、非感染组CRP、IL-6测定阳性率差异有统计学意义(χ2分别为85.0、75.0,57.0、54.3,P均<0.05)。CD64、CRP、IL-6诊断重症感染的敏感性、特异性分别为89.21%、78.35%,60.24%、67.56%,9.98%、71.02%,对所有感染诊断的敏感性、特异性分别为67.51%、82.45%,53.21%、74.62%,58.89%、80.23%。结论:细菌感染时CD64水平明显升高,全身性细菌感染时升高尤为明显,与CRP、IL-6相比诊断优越性更明显,特别对重症感染患儿诊断更有价值,可以作为早期诊断儿科感染性疾病的实验室依据。Objective: To investigate the value neutrophil cell surface markers CD64,CRP,IL-6 in pediatric infectious diseases.Methods:A total of 116 cases were divided into three groups: 49 patients with severe infection,36 cases with general infection,and 31 cases of non-infectious.CD64,CRP,IL-6 levels were measured and compared.Results: CD64 was highest in severe infection group,followed by general infection group and non-infection group,there was significant difference among three groups(H=35.02,P0.01).There was significant difference in positive detection rate of CRP and IL-6(χ2=85.0,75.0,57.0,54.3,all P0.05).Sensitivity of CD64,CRP and IL-6 were 89.21%,60.24%,9.98%;which specificity were 78.35%,67.56%,and 71.02%,respectively.Conclusions:During bacterial infection CD64 level significantly increases,especially during systemic bacterial infection.CD64 is more valuable than CRP and IL-6 in diagnosis,especially in severe infection diagnosis.It can be used as evidence in early diagnosis of pediatric infectious diseases.
