清肺通腑合剂对慢性呼吸衰竭患者胃肠功能障碍改善作用的临床研究

摘　　要：目的:评价清肺通腑合剂对慢性呼吸衰竭患者胃肠功能障碍的改善作用。方法:将慢性呼吸衰竭伴胃肠功能障碍的慢性阻塞性肺疾病急性发作患者46例随机分为莫沙比利对照组(23例)和清肺通腑合剂观察组(23例)。于实验开始前以及用药7天后对患者进行胃肠功能评分、急性生理学与慢性健康状况评分系统Ⅱ(APACHEⅡ)评分、血清白细胞介素-8(IL-8)、肿瘤坏死因子-α(TNF-α)的测定,治疗28天后观察患者的转归情况。结果:两组患者治疗后胃肠功能评分、APACHEⅡ评分、血清IL-8、TNF-α水平均有所降低,而肠鸣音较前提高,差异均有显著性(P<0.05);清肺通腑合剂治疗组在治疗后上述各项指标改善明显优于对照组(P<0.05)。结论:清肺通腑合剂对慢性呼吸衰竭患者胃肠功能障碍具有保护作用,明显改善患者预后。Objective: To evaluate the effect of qingfei tongfu mixture on gastrointestinal dysfunction in patients with chronic respiratory failure. Methods: 46 cases with acute exacerbation of chronic obstructive pulmonary disease who showed chronic respiratory failure and gastro- intestinal dysfunction at the same time were randomly divided into two groups,control group ( 23cases) was given mosapride,observed group ( 23cases) was given qingfei tongfu mixture,each group was given the corresponding treatment. The gastrointestinal function score and APACHE II score were recorded before and after treated for 7 days,serum IL - 8 and TNF - α levels were determined by ELISA. Results: The gastrointestinal function score and APACHEII score were obviously lowered in the two groups after treated,serum IL - 8 and TNF - α levels in the two groups were significantly decreased after treated. The improvement effects of qingfei tongfu mixture group were obvious better than those in the control group. Conclusion: Qingfei tongfu mixture can significantly improve the gastrointestinal dysfunction in patients with chronic respiratory fail- ure,obviously improve patients’prognosis.
