从经筋气血理论指导针刀治疗神经根型颈椎病的临床研究

摘　　要：目的:在经筋气血理论的指导下,运用针刀疗法治疗神经根型颈椎病,评价针刀疗法治疗神经根型颈椎病的有效性和安全性。方法:将60例神经根型颈椎病患者随机分为治疗组和对照组,治疗组30例采用针刀疗法,每周1次,共治疗2次;对照组30例采用牵引结合低频电治疗,每天1次,5次为1疗程,共进行2个疗程。观察治疗前后两组疼痛改善情况,NDI指数改善情况。结果:治疗组和对照组在治疗前疼痛评分、NDI评分无显著性差异(P>0.05);治疗后,疼痛评分、NDI评分治疗组和对照组与治疗前相比均有显著性差异(P<0.01):治疗后两组疼痛评分、NDI评分相比有显著性差异(P<0.01和P<0.05)。治疗组和对照组总有效率分别96.67%和83.33%:两组总有效率分别比较,治疗组均优于对照组(P<0.05)。结论:在经筋气血理论指导下,针刀疗法可显著缓解神经根型颈椎病患者颈肩部的疼痛等临床症状,其疗效显著高于对照组。Objective：To treat cervical spondylotic radiculopathy by knife needle therapy according to meridian tendons,Qi and Blood theory,and to observe the clinical efficacy and safety to treat cervical spondylotic radiculopathy.Methods：60 patients were divided into two groups：treatment group in which 30 cases were treated by knife needle therapy,with once a week for 2 times,control group in which 30 cases were treated by traction therapy and low-frequency therapeutic equipment,with once a day for 5 days,2 courses of treatment.Pain score and NDI index were observed in treatment group and control group before and after treatment.Results：There was no significant difference in treatment and control groups in pain score,NDI score before treatment（P0.05）.After treatment,there had a significant depress in pain score and NDI score compared with before treatment both in treatment and control groups（P0.01）.After treatment,the pain score and NDI score depressed more significantly in treatment group than in control group（P0.01 and P0.05）.In treatment group and in control group,the total effective rate was 96.67%and 83.33%,the total effective rate in treatment group was better than that in control group（P0.05）.Conclusion：Under the guidance of the meridian tendons,Qi and Blood theory,knife needle therapy can significantly relieve neck and shoulder pain and other symptoms of cervical pondylotic radiculopathy patients,and can improve cervical activity,the efficacy is significantly higher.
