甲型H1N1流感109例危重症病例临床流行病学资料回顾性分析

摘　　要：目的通过分析甲型H1N1流感危重症病例的临床流行病学和临床特征,为评价甲型H1N1流感的危害程度,预防和控制流感疫情提供科学依据。方法回顾性分析109例甲型H1N1流感危重症病例的临床流行病学、临床表现及治疗资料。结果 109例甲型H1N1流感病例中重症82例,危重症27例,死亡12例,病死率11.01%;发病以小年龄组为主,年龄中位数是4岁,职业以散居儿童为主,占42.20%;发病后前3天即就诊的病例占84.40%;35.78%的病例发病前有慢性基础病,所有危重症病例发病后均出现并发症,94.49%的重症病例发病后出现并发症;85.32%的病例采用神经氨酸酶抑制剂类抗病毒药物治疗,累积服用天数平均为4.9 d。结论甲型H1N1流感危重症病例起病急、症状重,早诊断、早期积极合理治疗,绝大多数均能获得痊愈。婴幼儿、有基础性疾病患者是预防控制的重点人群。[Objective]By analyzing the epidemiological and clinical features of patients with severe influenza A（H1NI）,to assess the damage degree of influenza A（H1NI）,provide a scientific basis for preventing and controlling the influenza. [Methods]The data of clinical epidemiology,clinical manifestation and treatment of 109 patients with severe influenza A（H1NI） were analyzed retrospectively. [Results]Among 109 patients with influenza A（H1NI）,there were 82 severe cases,27 critical cases and 12 dead cases,and the mortality was 11.01%.Most of cases occurred in younger age group,the median age was 4 years old,and 42.20% of cases were scattered children.84.40% of cases visited hospital within the three days after the onset.35.78% had basic chronic diseases before the onset.All critical cases had complications after the onset,and 94.49% of severe cases had complications.85.32 % of patients were treated by antiviral drugs,such as neuraminidase inhibitors.The average duration of administration was 4.9days. [Conclusion]The critical cases of influenza A（H1NI） occur urgently,and the symptoms are serious.Most cases can be cured by timely diagnoses and treatments.The key population of prevention and control are infants and people with basic chronic diseases.
