淮安市实施结核病防治规划(2001—2010)卫生经济学评价

摘　　要：目的评价淮安市实施《全国结核病防治规划(2001—2010年)》(简称《规划》)的卫生经济学效应,指导下一步卫生资源投入与使用。方法按照《全国结核病防治规划(2001—2010年)终期评估实施方案》的标准和要求,评估淮安市实施《规划》以来的社会效果、社会效益、社会成本、成本效应/效益成本比。结果 2001—2010年共投入1 830.8万元;发现活动性肺结核患者32 687例,其中新发涂阳13 401例(治愈率93.13%),复治涂阳2 079例(治愈率76.72%);减少肺结核患者死亡8 002人,避免新感染126 074人,避免新发肺结核12 607人;节约医疗费用858.85万元,挽回伤残调整生命年(DALY)188 384.9个,挽回社会间接经济损失17.92亿元;成本效应比97.18,效益成本比98.34。结论通过实施《规划》,实现高发现率、高治愈率的目标,减轻了患者的经济负担,避免了大量的经济损失,有效地保护了劳动力,取得了显著的社会效益和经济效益:下一步需进一步加大投入,实现更大的效益。[Objective]To evaluate the health economic effect of the implementation of the National Tuberculosis Prevention and Control Program（2001-2010） in Huai＇an City,guide the investment and utilization of the health resources in the future.[Methods]According to the standards and requirements of the Final Assessment Program for National Tuberculosis Prevention and Control Program（2001-2010）,the social effect,social benefits,social costs,cost-effectiveness/benefit-cost ratio since the program had been implemented were evaluated.[Results]18.308 million RMB had been invested during 2001-2010.A total of 32 687 active pulmonary tuberculosis cases were reported,of which 13 401 cases were new smear-positive tuberculosis cases with the cure rate of 93.13% and 2 079 cases were retreatment smear-positive tuberculosis cases with the cure rate of 76.72%.8 002 fewer deaths from tuberculosis were attributable to the Program,126 074 people were avoided to being infected by tuberculosis,and 12 607 people were avoided to being new tuberculosis patients.It saved medical expenses of 8.588 5 million RMB,retrieved disability-adjusted life years（DALY） of 188 384.9,and saved indirect social loss of 1.792 billion RBM.The cost-effectiveness ratio was 97.18,and benefit-cost ratio was 98.34.[Conclusion]By implementing the Program,the target of high detection rate and high cure rate has been achieved,the financial burdens of patients are reduced,economic loss is avoided obviously,the labor is effectively protected,and it has made significant social and economic benefits.In the future,it is required to further increase investment and achieve greater efficiency.
