子宫瘢痕部位妊娠保守治疗的临床分析

摘　　要：目的比较瘢痕部位妊娠两种保守治疗方法的临床效果。方法对2008年6月至2011年9月于我院收治的瘢痕部位妊娠实行保守治疗的32例患者的临床资料进行回顾性分析。并分为2组:单纯用药组15例和联合清宫组(用药后3天在超声监测行清宫术)17例,比较2组治疗后血β-HCG恢复正常时间、血细胞变化情况、转经时间。结果两组均有一例保守治疗失败改行手术治疗。联合清宫组血β-HCG下降较快,一周后较基础值下降78%,以后下降较缓慢约4周降至正常水平。单纯用药组血β-HCG下降较缓慢,约9周左右降至正常。联合清宫组治疗后转经时间恢复较单纯用药组快。治疗后2周两组血细胞变化均无明显差异。结论瘢痕部位妊娠保守治疗方法应个体化,单纯用药疗效确切,但用药后加清宫术治疗方法比单纯用药效果好,缩短了血β-HCG恢复正常时间,月经恢复也较快。Objective To compare tne clinical effects two kinds of methods of conservative treatment of scar pregnancy.Methods 32 patients who was treated by conservative method from June 2008 to September 2011 in our hospital were retrospectively analyzed.And it was divided into two groups： simple drug treatment was 15 cases and combined cleaning up the uterine cavity group was 17 cases（3 days after medication in the ultrasound monitor curettage）,two groups were compared after treatment with serum β-HCG returned to normal time,changes in blood cells,transfer through time.Results There were one case of failure of conservative treatment are diverted to surgery.The serum β-HCG decreased rapidly in cleaning up the uterine cavity group,after a week down 78% compared with baseline,then it down slowly about 4 weeks to normal levels.The serum β-HCG decreased slowly in simple drug treatment group,about 9 weeks to normal.The time to recover menstrual after treatment was short in combined cleaning up the uterine cavity group than that in simple drug treatment group.There was no significantly different in the change of blood cell after 2 weeks treatment.Conclusion It should be individualized in the conservative treatment of scar pregnancy,simple drug is effective,but combined cleaning up the uterine cavity after medication was more effective,because it shorten the recovery time of serum β-HCG,and the recovery period was also faster.
