盐酸戊乙奎醚术前用药对妊娠高血压综合征患者的影响

摘　　要：目的 观察盐酸戊乙奎醚于麻醉前用药对于妊娠高血压综合征（PIHS） 剖宫产患者的安全性和有效性.方法 50例PIHS患者随机分为盐酸戊乙奎醚组（Ⅰ组）和东莨菪碱组（Ⅱ组）,分别肌内注射盐酸戊乙奎醚0.5 mg和东莨菪碱0.3 mg,观察并记录给药前及给药后10、20、30 min时患者心率、血压、脉搏氧饱和度（SpO2）、呼吸频率、口干程度（VAS评分）、唾液分泌量变化情况.结果 与用药前比较：Ⅰ组心率、血压有轻微的改变,但差异无统计学意义（P〉0.05）;Ⅱ组心率、血压升高,差异有统计学意义（P〈0.05）.2组呼吸频率、SpO2比较无明显差异（P〉0.05）.口干程度VAS评分均高于给药前（P〈0.05）,但组间差异无统计学意义（P〉0.05）.2组唾液量均少于给药前（P〈0.05）.结论 术前使用盐酸戊乙奎醚对PIHS患者的心率、血压、SpO2、呼吸频率影响小,较东莨菪碱效果好,更适用于PIHS患者.Objective To evaluate the safety and efficacy of penehyclidine hydrochloride as premedication in patients with pregnancy - induced hypertension syndrome. Methods Fifty patients with pregnancy - induced hypertension syndrome undergoing operation of caesarean section were random!y divided into two groups ： group I was injected 0.5 mg penhvclidine hydrochloride intramuscularly, while group II was injected scopolamine 0.3 mg intramuscularly. The heart rate, blood pressure, pulse oxygen saturation, respiratory rate, visual analogue score and salivary volume of two groups were recorded before injection and 10 min, 20 min, 30 min after injection. Results Comparison of before treatment, the blood pressure and heart rate of group Ⅰ were changed slightly without satisfactory differences, however increased significantly in group Ⅱ. The respiratory rate and pulse oxygen saturation of two groups were changed slightly without satisfacto- ry differences. The visual analogue score of two groups were increased significar/tly compared with before treatment, but there were no satisfactory differences in two groups. The salivary volume of two groups were less than the before treatment. Conclusion Penhyclidine hydrochloride has small effects on heart rate, blood pressure, respiratory rate and pulse oxygen saturation in patients with pregnancy - induced hypertension syndrome, elatively good effect of scopolamine. It can be better used for pregnancy = induced hypertension syndrome patients＇ as premedication.
