慷舒灵敷料联合前列地尔注射液治疗糖尿病足的临床效果

摘　　要：目的研究慷舒灵敷料联合前列地尔注射液治疗糖尿病足的临床效果。方法筛选89例糖尿病足患者,按照随机数字方法分成2组。均严格控制血糖,在此基础上,对照组(n=44)予以常规换药;而研究组(n=45)在伤口上予以慷舒灵敷料局部换药,同时静脉使用前列地尔注射液治疗。完成规定疗程随访后,比较分析两组患者的相关临床资料。结果治疗后,研究组的尿微量白蛋白水平较对照组显著降低(P<0.05);研究组的Wagner病变分级较治疗前明显改善,且Wagner病变分级Ⅰ级和Ⅳ级例数较对照组明显减少(P<0.05);研究组的TC、TG、Cr、ALT及纤维蛋白原水平均较对照组明显改善(P<0.05);研究组的下肢动脉峰流速度和平均速度均较对照组明显提高(P<0.05);研究组的总有效率(95.56%)较对照组(72.73%)明显提高(P<0.05)。结论慷舒灵敷料联合前列地尔注射液具有较好的改善循环和抗菌作用,是治疗糖尿病足的一种有效的新方法。Objective To investigate the clinical effects of silvasorb combined with alprostadil fat emulsiom injection on patients with diabetic foot.Methods Eighty nine patients with diabetic foot were selected and randomly divided into two groups.Based on strict control of blood sugar,the control group（n=44）received conventional change of dressing,and the research group（n=45）received the local change of dressing combined with the intravenous transfusion of alprostadil fat emulsiom injection.After the course of treatment and the follow up,the clinical data of the two groups were compared and analyzed.Results After the treatment,the urinary microalbumin level of the research group decreased significantly,compared with that of the control group（P〈0.05）.In the research group,the grades of Wagner lesion were significantly improved compared with those before the treatment.And the cases of Ⅰand Ⅳ grades decreased significantly compared with those in the control group（P〈0.05）.The TC,TG,Cr,ALT and fibrinogen levels of the research group were significantly improved compared with those of the control group（P〈0.05）.Compared with the control group,the lower limb arterial peak flow velocity and the average velocity all significantly increased（P〈0.05）.The total effective rate of the research group was 95.56%,which was significantly higher than that of the control group（72.73%）（P〈0.05）.Conclusion Silvasorb combined with alprostadil fat emulsiom injection has good effects of improving the circulation and antibacterial activity,which is a new approach to the treatment of diabetic foot.
