摘  要:目的及方法:采用发色底物法检测冠心病血瘀症和非血瘀证患者血浆和外周血单核细胞t-PA、PAⅠ -1活性,研究并比较冠心病血瘀证患者血浆和单核细胞水平纤溶活性的改变及其意义。80例冠心病患者作 为观察组,其中血瘀证组30例,非血瘀证组50例(包括痰浊证27例,气滞证23例),健康对照组20例。结果: 冠心病血瘀证患者血浆PAⅠ-1活性显著高于非血瘀证组(P<0.01);血瘀证和非血瘀证患者PBMC PAⅠ-1 活性无显著差异(P>0.05),均高于正常对照组(P<0.01);血瘀证患者血浆t-PA活性与非血瘀证患者无显著 差异(P>0.05);血瘀证患者PBMCt—PA显著低于非血瘀证组(P<0.01)。结论:冠心病血瘀证患者在血浆和 单核细胞两水平纤溶活性降低,改变的趋势相同,血液成份机能的改变亦参与冠心病血瘀证的形成。Purpose To explore the essential changes of Coronary Heart Disease(CHD) patients with blood stasis(CHDS), the expression of fibronolytic components(Activity of tissue type plasminogen (t-PA) and its inhibitor(PAI-1) of plasma and peripheral blood monocyte(PBMC) of CHD patients were determinated. Method: All the parameters were measured by chromogenic substrate method respectively among 80 patients with coronary heart disease(30 cases with bood stasis,27 cases with phlegm-damp and 23 cases with qi-stagnation.20healthy persons were recruited as normal control group. Result:The plasma activity of PAI-1 was significantly higher in the patients of coronary heart disease with blood stasis syndrome(CHDS) than that in patients without blood stasis(NCHDS)(P< 0.01),no significant difference of PAI-1 activity of PBMC was found between CHDS group and NCHDS group(P> 0.05). PAI-1 activity of PBMC of both groups was significantly higher than that of the normal control group(p< 0.01 ) .there was no significant difference of plasma t-PA activity between CHDS group and NCHDS group(P>0.05) .PBMC activity of t- PA was significantly declined in the patients of CHDS group than that in NCHDS group(p<0.01 ). Conclusion: Fibrinolytic activity was reduced in the patients of CHDS group at plasma and monocyte levels, showing the changing trend. this indicates that altered function of blood components was the essential pathogenic factors for the development of CHDS.
