血尿酸/血肌酐比值对肾小管间质损害程度的评估价值

摘　　要：目的探讨血尿酸水平与肾小管间质损害程度的关系及血尿酸/血肌酐比值对肾小管间质损害程度的评估价值。方法共8种病理类型、1797例肾脏病患者入选,由于肾小管间质的评分方法不同,将患者分为IgA肾病组和非IgA肾病组,探讨肾脏病中血尿酸/血肌酐比值与肾小管间质损害程度的关系。结果血尿酸、血尿酸/血肌酐比值与肾小管间质损害程度相关(P<0.001)。比值升高提示轻度小管间质损害,普通人群组、IgA肾病和非IgA肾病小管间质病变0分组的血尿酸/血肌酐比值分别为3.66±0.02、4.20±0.06、3.63±0.10;比值降低提示中重度小管间质损害,普通人群组、IgA肾病小管间质病变2分组、非IgA肾病小管间质病变3分组的血尿酸/血肌酐比值分别为3.66±0.02、2.97±0.17、0.98±0.06。结论血尿酸/血肌酐比值对肾小管间质损害程度的评估有参考价值,比值升高或降低时均提示出现小管间质病变。Objective To investigate the association of serum uric acid levels with the degree of tubulointerstitial lesion, and the predictive value of serum uric acid/serum creatinine ratio for the degree of tubulointerstitial lesion. Methods 1797 cases from 8 kinds of biopsy-proven kidney diseases were enrolled and divided into IgA nephropathy group and none-IgA nephropathy group based on different classification of tubular atrophy/interstitial fibrosis. 1535 individuals from a community based cohort were enrolled as controls. Results Both serum uric acid levels and serum uric acid/serum creatinine ratio were associated with the degree of tubulointerstitial damage. Higher serum uric acid/serum creatinine ratio suggested slighter tubulointersfitial lesion. The creatinine levels of controls, TO of IgA nephropathy group and TO of none-IgA nephropathy group were 3.66±0.02, 4.20±0.06, 3.63±0.10 respectively, and lower serum uric acid/serum ereatinine ratio suggested severe damage. The serum uric acid/serum creatinine ratios of the controls, T2 of IgA nephropathy group and T3 of none-IgA nephropathy group were 3.66±0.02, 2.97±0.17, 0.98±0.06 respectively. Conclusions Serum uric acid/serum creatinine ratio can reflect the degree of tubulointerstitial damage. Both increased and decreased ratio suggest tubulointerstitial lesion.
