摘  要:目的研究α-硫辛酸对瓣膜置换术后患者肾功能的影响。方法将30例择期行瓣膜置换术的患者随机分为对照组(C组)和α-硫辛酸组(L组),每组15例。L组于体外循环(CPB)前静脉注入α-硫辛酸50mg/kg,术后l~5d均静脉滴注α-硫辛酸50mg/kg;C组静脉注入等容量生理盐水。分别于术前(T0)、术后第1天(T1)、术后第3天(T3)、术后第5天(T,)、术后第7天(T,)测定血清尿素氮(BUN)、血清肌酐(Cr)及血清p2微球蛋白(β2-MG)水平。结果两组患者血清β2-MG、Cr及BUN于术后T1、L均高于T0;相同时间点L组明显低于C组。结论围术期应用α-硫辛酸对瓣膜置换术后患者的肾功能具有保护作用。Objective To investigate the effect of lipoicacid on renal function in patients with cardiac valve replacement. Methods Thirty patients scheduled to receive cardiac valve replacement were randomly divided into lipoicaeid group(group L, n = 15) and control group(group C, n = 15). After tracheal intubation, lipoieacid 50 mg/kg was administered intravenously in group L and lipoicacid was infused at 50 mg/kg on the first 5 days after operation, while in group C only equivalent normal saline was administed. Blood samples were taken before operation(T0), on the lst(T1 ), 3rd(T3), 5th(T5), and 7th (T7) day after operation for measurement of serum urea nitrogen (BUN), creatinine (Cr), β2-mi- croglobulin ( β2-MG)- Results BUN, Cr and serum β2-MG levels increased significantly after operation at T1 and T3, as compared with the baseline values( TO )in both groups, but the levels in group L were significantly lower than those in group C at the same stage ( P 〈 0.01 ). Conclusions Applying li- poicacid at the perioperative stage has protective effects on renal function in patients with cardiac valve replacement.
