调节性T细胞及甲状腺自身抗体检测在自身免疫性甲状腺疾病中的应用

摘  要:观察Graves病(Graves disease,GD)、桥本甲状腺炎(hashimoto thyroiditis,HT)患者外周血调节性T细胞(regula-tory T cells,Treg)和叉头状转录因子FoxP3的表达情况及与抗甲状腺球蛋白抗体(TGAb)、抗甲状腺过氧化物酶抗体(TPOAb)表达的关系。用流式细胞术检测GD、HT患者外周血Treg、FoxP3的表达情况并与正常人群进行比较,并同时检测TGAb、TPOAb,观察FoxP3与TGAb、TPOAb之间的相关性。结果:GD组患者、HT患者CD4+CD25+Treg细胞数量与健康对照组相比无差异(P>0.05);而GD组患者、HT患者外周血中CD4+CD25+FoxP3+比例与对照组比较均存在着显著性差异(P<0.05)。对照组和AITD患者的TGAb、TPOAb水平存在显著性差异(P<0.05),且HT患者TGAb、TPOAb水平比GD组高(P<0.05)。Foxp3与TGAb(r=-0.623,P=0.000)、TPOAb(r=-0.555,P=0.000)成负相关。TGAb、TPOAb对于诊断和鉴别AITD具有重要意义,是HT诊断和鉴别诊断的首选指标。Treg及Foxp3在AITD的发生发展中起到重要作用,但Foxp3表达与AITD更有相关性。To observe expression of regulatory T-cells (Treg) and Foxp3 and their correlation with TGAb and TPOAb in patients with GD and HT, flow cytometry was used to analyze the expression of Treg and Foxp3 on peripheral blood lymphocyts of the patients, and its correlation with TGAb and TPOAb was analysed. There was no significant difference in the number of CD4^+ CD25^+ T between HT and GD patients with the normal controls (P〉0.05), whereas the expression of Foxp3 was significantly decreased in GD, HT patients compared with that in the controls(P〈0.05). The levels of TGAb, TPOAb were significantly increased in GD and HT patients compared with those in the controls(P〈0.05), but much higher in HT patients than in GD patients(P〈0.05). A significant negative correlation between Foxp3 levels and TGAb(r = -0. 623, P= 0. 000) and TPOAb(r =-0. 555, P=0. 000)was found. Taken together, these results indicate that TGAb, TPOAb are important for the differential diagnosis of HD, and the expression of Foxp3 may better related to AITD than Treg.
