CDAI和SDAI评估类风湿关节炎的比较研究

摘　　要：目的证实临床的疾病活动指数(CDAI)、简化的疾病活动指数(SDAI)与疾病活动指数28(DAS28)的相关性,初步探讨CDAI和SDAI对于RA的评估。方法连续纳入220例RA患者,评估每例RA患者的关节肿胀数(SJC)、关节压痛数(TJC)、患者对病情的总体评估(PGA Vas)、医生对病情的总体评估(PhGA Vas)、检测血沉(ESR)、C-反应蛋白(CRP),计算DAS28、CDAI、SDAI。用Pearson相关分析DAS28、CDAI、SDAI、HAQ任意两者的相关性,并用多重线性回归分析SDAI各组成部分的影响。结果 220例RA患者的CDAI(17.190±11.081)、SDAI(19.075±11.570)与DAS28(4.314±1.492)均呈正相关,相关系数r=0.876、0.861。SDAI各组成部分中,炎性指标CRP对其影响较小(β=0.216)。结论 SDAI和CDAI同DAS28一样,亦可以用于评估RA的活动性。目前以实现缓解为RA的达标治疗,SDAI和CDAI可以更准确的评估RA的治疗效果。相比于SDAI,CDAI缺少CRP,可随时随地用于RA的评估,更适用于RA的日常评估。Objective To testify the correlation of Clinical disease activity index（CDAI） and simplified disease activity index（SDAI） as well as Disease activity score 28（DAS28）.Investigate initially the assessment of CDAI and SDAI for RA.Methods Intake 200 rheumatoid arthritis（RA） patients and record swelling joint counts（SJC）,tenderness joint counts（TJC）,erythrocyte sedimentation rate（ESR）,C reaction protein（CRP）,patient＇s and doctor＇s global assessment of disease activity based on visual analogue scale of each patient.DAS28,CDAI,SDAI of all patients are calculated by their formula and all patients.Statistical evaluation was carried out by Pearson correlation,multiple linear regression analysis.Results Of all the patients,CDAI（17.190±11.081） and SDAI（19.075±11.570） are correlated with DAS28（4.314±1.492）,the correlation coefficients are 0.876,0.861.The standard partial regression coefficients of TJC,SJC,PGA,MDGA,CRP are 0.404,0.300,0.197,0.234,0.216.The influence of CRP is weaker compared with other components of SDAI.Conclusion CDAI and SDAI as well as DAS28 can assess the activity of RA well.What＇s for,SDAI and CDAI can assess the remission of RA,as now remission is our treatment goal.Though CDAI is short of CRP compared with SDAI,CDAI can assess RA regardless of the time and place.So CDAI is suitable for daily application better.
