动力髋螺钉结合松质骨螺钉治疗老年股骨转子间骨折  

Study of Dynamic Hip Screw Fixation Combined with Adjunctive Cancellous Screw in the Treatment of Intertrochanteric Fractures

摘　　要：目的：探讨动力髋螺钉（DHS）结合松质骨螺钉治疗老年股骨转子间骨折的疗效。方法：2007年3月-2008年10月,采用DHS结合松质骨螺钉内固定方法治疗65例老年股骨转子间骨折患者。术后随访10~22个月。采用Kudema改良Merled’Aubigne标准评价髋关节功能。结果：术后无切口感染,无死亡病例,无骨折不愈合,无内固定松动、断裂及严重移位等并发症。发生髋内翻2例。骨折临床愈合时间为15~23周,平均18周。髋关节功能评价优39例,良19例,可4例,差3例,优良率为89.2%。结论：DHS结合松质骨螺钉内固定是治疗老年股骨转子间骨折一种较为满意的手术方法。Objective：To investigate therapeutic effect of dynamic hip screw（DHS） fixation combined with adjunctive cancellous screw for intertrochanteric fractures.Methods：65 old patients with intertrochanteric fractures were treated by DHS fixation combined with adjunctive cancellous screw from March 2007 to October 2008.Hip functions were evaluated using Merled’Aubigne criterion. Results：All the patients were followed up for 10 to 22 months after operation.Fracture healing was achieved within 15 weeks to 23 weeks,with an average of 18 weeks.According to the Merled’Aubigne criterion,the excellent and good rate of hip function was 89.2%（excellent in 39 cases, good in 19 cases, fair in 4 cases and poor in 3 cases）. Complications such as loosening, breakage,or grievous migration of hardware were not observed beside coxa vara of 2cases.Conclusion：DHS fixation combined with adjunctive cancellous screw is a effective method for the treatment of intertrochanteric fractures.
