两种方法对巨细胞病毒-IgM抗体水平检测敏感性和特异性的比较

摘　　要：目的:比较ELISA和realtime-PCR(RT-PCR)对产前筛查女性巨细胞病毒-IgM抗体水平检测的敏感性及特异性。方法:收集146例育龄期行产前筛查女性,采用ELISA和RT-PCR法评估所有入选者外周血巨细胞病毒-IgM抗体水平,同时比较这两种检测方法的敏感性和特异性。结果:(1)ELISA法的敏感性为64.6%,RT-PCR敏感性为72.7%,差异有统计学意义(P<0.05);(2)ELISA法的特异性为54.8%,RT-PCR特异性为60.8%,差异有统计学意义(P<0.05);(3)ELISA法检测费用为每个标本(68.5±4.3)元,采用RT-PCR检测为(71.6±5.5)元,两组间比较差异无统计学意义(P=0.163)。结论:与ELISA相比,采用RT-PCR检测巨细胞病毒-IgM抗体水平具有较高的敏感性和特异性,且不增加费用支出,值得临床推广。Objective：To compare the sensitivity and specificity of ELISA and real time-PCR（RT-PCR）on the detection of cytomegalovirus-IgM（CMV-IgM）antibody concentration in women with pre-pregnant screening.Methods：146 women underwent pre-pregnant screening were enrolled and peripheral blood were drawn to evaluate the concentration of CMV-IgM antibody with the methods of ELISA and RT-PCR.the sensitivity and specificity of ELISA and RP-PCR on the detection of CMV-IgM antibody concentration were compared concomitantly.Results：（1）The sensitivity of ELISA was 64.6% while RT-PCR was 72.7%,and the difference was statistically significant（P0.05）.（2）The specificity of ELISA was 54.8% while RT-PCR was 60.8%,and there was statistically significant（P0.05）.（3）The expenditure was（68.5±4.3）RMB in each sample detection with ELISA while it was（71.6±5.5）RMB with RT-PCR,and there was no significant difference（P=0.163）.Conclusion：Compared to the ELISA,the sensitivity and specificity of CMV-IgM antibody concentration evaluation was higher with RT-PCR,and RT-PCR did not increase the expenditure which was deserved to be broadly applied.
