摘  要:目的观察阵发性睡眠性血红蛋白尿症(PNH)患者骨髓正常造血克隆(CD34+CD59+)和异常造血克隆(CD34+CD59-)细胞在粒细胞集落刺激因子(G—CSF)或干细胞因子(SCF)体外刺激后胞质内信号转导及转录激活子5(STAT5)磷酸化表达差异。方法26例PNH患者及14名健康对照骨髓单个核细胞(BMMNC)经和不经100ng/mlG—CSF、SCF刺激,应用蛋白磷酸化流式细胞术分别检测其中CD34+CD59+和CD34+CD59-细胞磷酸化STAT5(P—STAT5)的平均荧光强度(MFI)。结果(1)细胞因子刺激前,PNH患者CD34+CD59-细胞内P—STAT5的MFI(单位:道数)为(24±18),明显低于CD34+CD59+细胞(644-49)和健康对照CD34+CD59+细胞(614-33)(均P〈0.01);后两者间差异无统计学意义(P〉0.05)。(2)G—CSF刺激后,PNH患者CD34+CD59-细胞P-STAT5的MFI为(36±35),明显低于CD34+CD59+细胞(124±84)和健康对照CD34+CD59+细胞(116±59)(均P〈0.01);后两者间差异无统计学意义(P〉0.05)。(3)SCF刺激后,PNH患者CD34+CD59-细胞P—STAT5的MFI为(34±27),明显低于CD34+CD59+细胞(124±97)和健康对照CD34+CD59+细胞(128±62)(均P〈0.01);后两者间差异无统计学意义(P〉0.05)。(4)G—CSF或SCF刺激后,PNH患者及健康对照CD34+CD59+细胞P—STAT5的MFI较刺激前的增高程度,均明显超过PNH患者CD34+CD59-细胞(均P〈0.01)。结论PNH克隆在体外应用G—CSF、SCF刺激后,胞内信号传导(STAT5)磷酸化明显低于正常造血克隆。Objective To study the expressions of phosphorylated STATS (P-STATS) in CD34 + CD59- and CD34 + CD59 + bone marrow cells of the patients with paroxysmal nocturnal hemoglobinuria (PNH) before and after in vitro G-CSF or SCF stimulation, then evaluate the functions of G-CSF and SCF receptors in PNH clone ceils. Methods Bone marrow mononuelear cells (BMMNC) of 26 PNH patients and 14 normal controls were stimulated in vitro with G-CSF (100 ng/ml) or SCF (100 ng/ml) for 10 min. Before and after these stimulations, the mean fluorescence intensity (MFI) of P-STATS in CD34 + CD59 + BMMNC and CD34 + CD59 - BMMNC were measured by flow cytometry. Results ( 1 ) The P-STATS MFI was (24 ± 18 ) in unstimulated CD34 + CD59 - cells. And it was significantly lower than that in unstimulated CD34 + CD59 + cells of PNH patients ( 64 ± 49 ) and normal controls ( 61± 33 ) ( both P 〈 0.01 ). No statistic difference existed between the latter two. ( 2 ) The P-STATS MFI was ( 36 ± 35 ) in G-CSF stimulated CD34 + CD59 - cells of PNH patients. And it was significantly lower than that in G-CSF stimulated CD34 + CD59 + cells of PNH patients ( 124 ± 84) and normal controls ( 116 ± 59) ( both P 〈 0. 01 ). There was no statistic difference between the latter two. (3)The P-STATS MFI was (34 ± 27) in SCF stimulated CD34 + CD59 - cells of PNH patients. And it was significantly lower than that in SCF stimulated CD34 + CD59 + cells of PNH patients ( 124 ± 97 ) and normal controls ( 128 ±62 ) ( both P 〈 0. 01 ). And no statistic difference existed between the latter two. (4)The increased P-STATS MFI in G-CSF stimulated CD34 + CD59- cells of PNH patients was significantly lower than that in G-CSF stimulated ED34 + CD59 + cells of PNH patients and normal controls ( both P 〈 0. 01 ).
