摘　　要：目的探讨甲胎蛋白(AFP)浓度、α-L-岩藻糖苷酶(AFU)活性、5′-核苷酸酶(5'-NT)活性在原发性肝癌诊断和治疗中的应用价值。方法应用化学发光法和速率法对原发性肝癌患者血清AFP、AFU和5'-NT含量进行检测,并分别与良性肝病组和健康对照组进行比较。结果原发性肝癌患者血清AFP、AFU和5'-NT含量显著高于良性肝病组和健康对照组,差异有统计学意义(P<0.01)。AFP、AFU和5'-NT在原发性肝癌中的敏感性分别为68%、81%、80%。AFP和AFU,AFP和5'-NT,AFP、AFU和5'-NT联合检测敏感性分别为88.8%、87.2%、92.0%。治疗前AFP、AFU和5'-NT含量显著高于健康对照组,差异有统计学意义(P<0.05),治疗后6个月,复发者又恢复到治疗前水平,未复发者居正常水平。结论测定原发性肝癌患者血清中AFP、AFU和5'-NT活性变化对肝癌患者诊断和治疗预后均有重要临床价值。Objective To explore the application value of alpha-fetoprotein（AFP）,alpha-L-fucosidase（AFU） and 5′-nucleotidase（5＇-NT） in the patients with primary hepatic cancer（PHC）.Methods Serum AFP,AFU and 5′-NT were detected with the chemiluminescence and speed methods in the patients with PHC and compared with the benign liver disease group and healthy control group.Results The serum AFP,AFU and 5＇-NT levels were significantly higher in the patients with PHC than the patients with benign liver diseases and healthy controls with statistical difference（P0.01）.The sensitivities of AFP,AFU and 5＇-NT in primary liver cancer group were 68%,81% and 80% respectively.The sensitivities of combined measurement of AFP plus AFU,AFP plus 5＇-NT and AFP plus AFU and 5＇-NT were 88.8%,87.2% and 92.0% respectively.Before therapy,serum levels of AFP,AFU and 5＇-NT in the PHC group were significantly higher than those in the healthy control group with statistical difference（P0.05）.In 6 months after treatment,the levels in the patients without recurrence remained normal.However,the levels in the patients with recurrence returned to those before therapy again.Conclusion The detection of activity changes of serum AFP,AFU and 5＇-NT in the patients with PHC has important value in the diagnosis,treatment and prognosis of PHC.
