摘  要:目的探讨微创锁定钢板内固定治疗股骨远端C型骨折的疗效。方法应用锁定钢板治疗股骨远端C型骨折41例,随机分为微创组20例,对照组21例应用切开复位锁定钢板内固定。记录比较2组手术时间、手术出血量、切口长度、植骨例数、骨折愈合时间、膝关节HSS评分、膝关节活动度。结果微创组与对照组平均手术时间分别为85.5±14.95分钟和103.33±18.99分钟,术中出血量分别为86.5±81.58 ml和228.10±167.08 ml,切口长度分别为13.08±1.48 cm和18.90±2.72 cm,骨折愈合时间分别为15.6±2.78周和18.48±3.67周,HSS评分分别为85.95±6.43分和80.71±7.31分,膝关节活动度分别为130.45±5.78°和111.48±7.95°。2组手术时间、术中出血量、切口长度、骨折愈合时间、HSS评分、膝关节活动度比较均有统计学差异﹙<0.05﹚。微创组有5例术中植骨,有2例发生5°以上膝关节内外翻畸形;对照组有14例术中植骨,有2例发生5°以上膝关节内外翻畸形。结论微创锁定钢板内固定能很好的完成股骨远端C型的手术治疗,可以获得满意的临床效果。Objective To observe the efficacy of the minimally invasive percutaneous plate osteosynthesis in the treatment of the distal femoral C type fractures.Methods 41 cases of distal femoral fractures were treated with the locking proximal humerus plate.There were randomly divided into 2 groups: minimally invasive percutaneous plate osteosynthesis in 20 cases﹙minimally invasive group﹚,and the traditional anteromedial incision approach in 21cases﹙traditional group﹚.All fractures were fixated with the locking proximal humerus plates.The operation time,the blood loss,the incision length,the healing time of bone,the HSS score,the motion of the knee joint were recorded.All the data were analyzed by SPSS17.0 statistical software.Results The mean operative time was 85.5±14.95 min and 103.33±18.99 min﹙ 0.05﹚ in minimally invasive group and raditional group,the blood loss was 86.5±81.58ml and228.10±167.08 ml﹙ 0.05﹚in minimally invasive group and raditional group,the incision lengths were 13.08±1.48cm and 18.90±2.72cm﹙ 0.05﹚in minimally invasive group and raditional group,the fracture healing time was 15.6±2.78week and 18.48±3.67week ﹙ 0.05﹚in minimally invasive group and raditional group,the HSS score were 85.95±6.43 and 80.71±7.31﹙ 0.05﹚ in minimally invasive group and raditional group and the motion of the knee joint were 130.45±5.78 °and 111.48±7.95° ﹙ 0.05﹚in minimally invasive group and raditional group.Conclusion The treatment of distal femoral C type fracture with the minimally invasive percutaneous plate osteosynthesis is a perfectly innovational mothed.
