益气养阴活血法治疗早期糖尿病肾病血液流变学和血脂的临床研究

摘　　要：目的观察益气养阴活血法对早期糖尿病肾病的血流变学和血脂的影响。方法将符合纳入标准的早期糖尿病病人120例,随机分为治疗组和对照组各60例。两组均予糖尿病肾病基础治疗,治疗组予加有益气养阴活血作用的中药汤方(自拟方),对照组加服百令胶囊口服,两组均观察12周,测定两组治疗前和治疗后血脂、血液流变学的变化情况。结果①两组治疗前后血液流变学各指标比较,治疗组治疗后血液流变学各指标较治疗前均降低,差异有统计学意义(*P<0.05),对照组无明显变化(△P>0.05)。②治疗前后两组血脂比较,两组治疗后,总胆固醇(CHOL)低密度脂蛋白胆固醇(LDL-C)较治疗前下降,两组组内差异均具有统计学意义(★P<0.05),治疗组与对照组间变化不明显(*P>0.05)。治疗组治疗后甘油三脂(TG)较治疗前下降,差异具有统计学意义(★P<0.05)。对照组治疗组后无明显变化(P>0.05)结论益气养阴活血法在一定程度上能改善早期糖尿病肾病患者的血液流变学和高脂血症。Objective To evaluate the clinical effects on the patients with early diabetic nephropathy with the therapy of strengthing Qi, nourishing yin and promoting blood circulation. Methods 120 cases of patients with early diabetic nephropathy were divided into treating group and control group by 1 ： 1 ratio randomly. We gave the same basic treatment for all the patients. At the same time, we gave the decoction which has effect of strengthing Qi,nourishing yin and promoting blood circulation to treating group and Bai Ling capsule to control group. The Hemorrheology and blood lipids, liver and kidney function were compared before treatment and after 12weeks treatment. Results （!）Compared the change of hemorrheology before and after treatment, The signs of hemorrheology and serum fibrinogen were all decreased than that of before treatment in treating group,The difference was statistically significant （P 〈 0. 05 ）. （~）Compared the change of blood lipid before and after treatment, The low-density lipoprotein blood cholesterol were decreased in both group after treatment, The difference was statistically significant（P 〈0. 05）. triglyceride was apparently decreased in treating group, the difference was statistically significant（ P 〈 0. 05）. Conclusion To a certain extent, the therapy of strengthing Qi, nourishing yin and promoting blood circulation could better improve the hemorrheology,alleviate hyperlipidemia of patients with early diabetic nephropathy. It indicated that the effect of using therapy of the strengthing Qi, nourishing yin and promoting blood circulation together with the western medicine treatment was better than using the western medicine treatment together with Bailing capsule.
