超声引导用无水乙醇治疗女性盆腔良性囊性占位技术探讨

摘　　要：目的回顾分析超声引导下抽液无水乙醇固化治疗女性盆腔良性囊性占位的临床疗效,并探讨其技术细节。方法 418例患者430个囊肿进行超声引导穿刺抽液加无水乙醇凝固治疗。囊肿最大直径4.0～18.5 cm(平均7.2 cm),其中多房性38例(8.8%)。每次乙醇注入量为囊液量的2/3,最大上限100 ml,留置3 min后全部抽出,重复3次。结果本组病例手术成功率为100%,治愈率为98.8%。随访9～68个月,5例(1.2%)囊肿复发,包括1例(0.2%)巧囊和4例(0.9%)炎性包裹性积液。结论超声引导下穿刺抽液无水乙醇固化治疗女性盆腔良性囊性占位是一种创伤微小、临床效果可靠的技术。掌握操作过程中的技术细节是保证手术成功和有效,防止术后复发的关键。Objective To evaluate the effectiveness of ultrasound-guided aspiration and ethanol sclerotherapy in female patients with pelvie cystic mass, as well as summary to the technical details. Methods Four hundreds and thirty cysts with maximum diameter of 4.0- 18.5 cm（Mean 7.2 cm） in 418 patients were treated with ultrasound- guided aspiration and ethanol sclerotherapy. After aspiration of the cyst contents, approximately two thirds of the aspirated fluid was replaced with 100% ethanol for 3 minutes, and then aspirated. The same procedure was repeated for three times. Results The technical successful rate and cure rate are 100% and 98.8%, respectively. Five cysts （1.2 %） had recurred at follow-up examinations（9 - 68 months） which included one endometrioma（0.2 % ） and four inflammatory encapsulated fluid（0.9%）. Conclusions It is a simplicity and effectiveness technology for ultrasound guided aspiration and ethanol sclerotherapy of cysts in pelvic cavity. It is important to grasp technical detail about operation to ensure high technical effectiveness rate and low recurrent rate.
