摘  要:目的构建常见先天性心脏畸形(congenital heart disease,CHD)胎儿心脏超薄断层切面数据库,为建立常见CHD胎儿心脏三维模型和解剖图谱奠定基础。方法选用4例终止妊娠的常见CHD胎儿心脏标本,预处理后分别进行层厚为60μm的连续横切,同时用数码照相机微距拍摄,连续获取断层切面图,建立常见CHD胎儿心脏超薄断层切面数据库。结果本研究建立了层厚60μm、像素3 888×2 592、JPEG格式的法洛四联症、完全心内膜垫缺损、肺动脉闭锁、大动脉转位4种常见CHD胎儿心脏超薄断层切面数据库各1例。数据库图像清晰显示了房室腔、大血管及瓣膜等结构,维持了心脏和周围组织的位置关系,从不同层面显示了病变部位及程度,反映了不同类型胎儿CHD的病理解剖特征,和胎儿超声心动图(fe-tal echocardiography,FECG)有极好的对应性,同时也清晰显示了目前超声心动图技术不能显示的解剖结构,为亚型诊断提供了依据。结论本研究所建立数据库断层图像清晰,反映了不同类型及亚型胎儿CHD的病理解剖特征,为建立常见CHD胎儿心脏三维模型和解剖图谱奠定基础。Objective To establish the ultra-thin section databases of the malformed hearts of fetuses with common congenital heart disease (CHD) to lay the foundation for the construction of three-dimensional models and ana- tomical atlas of the fetal hearts. Methods This study chose the fetuses with common CHD who underwent termination of pregnancy. After pretreated, serial transverse sections of the malformed hearts with the thickness of 60μm were made and serial images of the sections were obtained by macro shooting through a digital camera to establish the ultra-thin section databases of the fetal hearts of common CHD. Results In this study, the databases of tetralogy of Fallot, complete endocardial cushion defect, pulmonary atresia and transposition of great arteries each one, with a thickness of 60 μm, high resolution (3 888× 2 592 pixels), JPEG format were established. The images of these databases displayed the fine structures of the fetal hearts such as the atrioventricular cavity, great vessels and cardiac valves clearly and maintained the positional relationship of the fetal heart to its surrounding tissues; The databases dis- played the location and severity of pathological changes at different sectioning levels and reflected pathological anatomical characteristics of different types of fetal CHD, which had excellent coincidence with fetal echocardiography. Mean- while, the databases also displayed the anatomical structures clearly that were impossible to be visualized by current echocardiography, providing the basis for subtype diagnosis. Conclusions The section images of the databases clearly displayed the pathological anatomical characteristics of different types of fetal CHD as well as their subtypes, which could lay the foundation for the construction of three-dimensional models and anatomical atlas of these fetal hearts.
