2009年大连开发区151例高血压患者社区自我管理模式效果分析

摘　　要：[目的]了解大连开发区高血压患者社区管理效果,为本地区建立科学、系统的高血压防治体系提供依据。[方法]2009年7月,在大连开发区湾里社区对151例高血压患者进行6个月的自我管理,并对管理效果进行分析。[结果]调查高血压患者151例,管理前后收缩压分别为154.3±44.3、153.4±43.4mmHg(P>0.05),舒张压分别为96.2±25.7、94.2±23.2mmHg(P>0.05)。高盐饮食的高血压患者比例管理前后分别为26.49%、15.23%(P<0.01),有时或经常锻炼、精神放松、通过增加锻炼控制体重、规则服药的比例管理前后分别为78.81%、29.14%、50.99%、19.87%和90.73%、39.74%、68.21%、36.42%(P<0.01或0.05);受教者对自我管理模式的认可比例、对家庭管理模式认可的比例管理前后分别为81.46%、19.21%和91.39%、43.71%(P<0.05或0.01);高血压相关知识知晓率明显提高(P<0.05)。[结论]对高血压患者开展自我管理模式收效显著,适合在该地区推广。[Objective]To understand the effect of self- management among patients with hypertension in a community, so as to provide the basis for establishing a scientific and organized hypertension prevention system. [Methods]Self-management was carried out for 6 months among 151 hypertensive patients in communities in Dalian Development Zone,then management effectiveness was evaluated. [Results]Of 151 patients with hypertension, the SBP before and after manage ment was 154.3±44.3 and 153.4±43.4 mmHg（ P 〉0.05）respectively,the DBP before and after management was 96.2±25.7 and 94.2±23.2 mmHg（ P 〉0.05）respectively. The proportion of those with high salt intake was 26.49% and 15. 23% （ P 〈0.01）before and after management,the proportion of those with sometimes or regular exercise,mental ease, control weight by exercise, regular medicine taking was 78.81%, 29.14%, 50.99%, 19.87% and 90.73%, 39.74%, 68.21%and 36.42 % （P 〈0.01 or 0. 05 ） before and after management respectively, the proportion of those affirmative with self-management, that of those affirmative with family management was 81.46 %, 19.21 % and 91.39 %, 43.71 % （ P 〈0.05 or 0.01）before and after management,the knowledge rate of hypertension of 151 patients had a marked advance before and after management （ P 〈0.05）. [Conclusion]The method of self-management among patients with hypertension in a community is effective,it can be promoted in this district.
